PURPOSE:

To ensure timely, efficient, and accurate delivery of equipment and/or supplies.

POLICY:

        Delivery requests shall be filled in a timely, efficient and accurate manner by qualified
         personnel seven (7) days a week.

        Rental orders shall be filled with equipment which is absolutely clean, free from functional
         defects and relatively free of major cosmetic defects. Delivery staff are responsible for
         assuring that these standards are met at the time of equipment delivery.

        Equipment and/or supplies shall be delivered to clients within 24 hours of the request unless
         mutually agreeable alternate arrangements have been made. Requested items not maintained
         in the inventory shall be secured within a reasonable period of time. The client and/or referral
         source shall be made aware of delays, if encountered.

        All equipment shall be loaded and restrained in such a way as to prevent unnecessary damage
         and avoid injury during transit.

        Delivery staff shall take appropriate care in the loading and unloading of equipment to prevent
         damage and/or injury to equipment, employee, client, caregiver or client’s property.

        An Incident Report shall be filed (see Incident Reporting policy) if:

                 Client’s property or furnishings are damaged

                 Equipment is damaged

                 Client is injured

                 Employee is injured

                 Employee is injured during the loading and unloading process

                 Any unusual circumstances occurs during delivery

        Delivery staff shall be instructed on good body mechanics and shall adhere to these principles
         when loading or unloading equipment.

        A daily record of deliveries shall be maintained and shall include the following:

                 Date

                 Truck number

                 Client name

                 Location, map reference

                 Type of service

                 Invoice number

                 Client’s phone number

                 Order taken by

                 Other information, as appropriate

PERSONNEL STANDARDS:

        Personnel who deliver equipment shall:

                 Be knowledgeable of the equipment and its operations

                 Understand the electrical requirements of the device, if applicable

                 Be knowledgeable of the safety requirements

                 Meet applicable DOT criteria

                 Observe the following infection control procedures:

                          Handle equipment as little as possible

                          Handle equipment in a manner that will protect it from contamination:

                                 Avoid touching the interior surfaces of equipment

                                 Avoid excess talking, coughing or sneezing while equipment surfaces are
                                  exposed

                          Utilize good hand washing techniques by:

                                 Cleansing hands with a germicidal hand cleaner before and after entering
                                  a client’s residence

                                 Wash with soap and running water as soon as feasible

                 Assure that all equipment has been tested and inspected for proper functioning prior to
                  delivery

                          At the delivery location, the equipment will again be tested and inspected for
                           proper functioning

PROCEDURE:

        Equipment to be delivered will be selected from the “Client Ready” equipment area.

        Prior to delivery the equipment will be inspected to verify that the preventive maintenance
         inspection has been completed and is current, that the equipment is clean and in good working
         order.

        Rental equipment is to be signed out with the serial number of the equipment and the client’s
         name, address and phone number.

        At the time of delivery, the following information and/or forms are explained to the
         client/caregiver and appropriate signatures obtained:

                 Authorization to Treat

                 Rights and responsibilities/DMEPOS Supplier Standards

                 Financial Responsibility

                 Client Handbook

                 Client Education Materials

        If the client is unable or unwilling to sign the required forms, delivery personnel are to notify
         their supervisor.

        Delivery personnel will perform setup or assembly of equipment only after they have received
         appropriate training to do so.

        All equipment is to be checked for proper functioning.

        Instructions regarding setup and use of equipment, and safety precautions are to be
         documented, along with the client’s/caregiver’s level of understanding.
