Guidance Notes for Accident, Incident and Condition Reporting
                                                SD0140
GENERAL:
1. All accidents, potentially dangerous (near miss) incidents and conditions must be
    reported on the Accident Form, “SD0130 Accident Form”.
2. A ‘condition’ is any symptom you believe to be work-related e.g. musculo-skeletal pain
    when using equipment,
3. Writing must be legible and in black ink,
4. A separate form must be completed for each person injured,
5. A single form may be used to report an incident where there is no apparent injury, this will
    include a list of all those involved,
6. Forms not completed satisfactorily will be returned,
7. Do not delay completing the form because you are unable to answer all sections,
8. Contact the Safety Officer straight away if you have problems completing the form,
9. Additional information may be written on a separate sheet and attached to the form,
10. Completing and signing this form does not constitute an admission of liability of
    any kind, either by the person making the report or any other person.


SECTION A and B:
1. To be completed by the person involved; the line manager may complete the form if the
person involved is not able to.
2. Description of Event: give a detailed but concise description about what happened, giving
any unusual circumstances.

SECTION C:
To be completed and signed by the line manager after investigating the accident. Attach
witness statements if appropriate.


COMPLETED FORMS:
Send completed forms to the Departmetal Safety Supervisor (DSS) within 48 hours.

DISTRIBUTION OF FORMS:
The DSS will photocopy and distribute the form as per the College Policy to
- Safety Office (original)
- Head of Department – usually kept by DSS on HOD’s behalf
- Occupational Health
- Person injured or involved if requested

If an undergraduate student send an additional copy to Academic Registry.
(Post-graduate students are regarded as staff, the DSS having a copy).
