JOB DESCRIPTION SPECIALTY DOCTOR IN MEDICINE TYRONE COUNTY HOSPITAL, OMAGH September 2009 TITLE: Specialty Doctor in Medicine SPECIALTY: Rehabilitation and Palliative Care BASE: Tyrone County Hospital, Omagh, Co Tyrone RESPONSIBLE TO: Dr E Hodkinson, Dr F Robinson. JOB SUMMARY To support the existing Consultant Physicians in Care of the Elderly and Palliative Care in the provision of a rehabilitation and palliative care service. To assist in the provision of an outpatient clinic for the assessment and management of Transient Ischaemic Attacks. This will be a full time post (10 PA’S). PROFILE OF HOSPITAL Tyrone County Hospital, Omagh is the local centre for rehabilitation. The Stroke and Rehabilitation Unit has 16 beds and provides rehabilitation for older people. In addition there are 10 beds for orthogeriatric rehabilitation and Palliative care. There are 14 beds for long stay care of the elderly in an area adjacent to the rehabilitation ward. Over time these long stay beds will become rehabilitation beds, making an overall complement of 40. The hospital has an Urgent Care and Treatment Centre which is doctor led and nurse delivered. The Sub regional Renal Dialysis Unit for the Western Board Area is centred in the hospital. The Cardiology Service at Tyrone County Hospital comprises a nurse-led observation and assessment area with a cardiac ambulance service. The Cardiac Investigations Department provides treadmill testing, echocardiography, 24 hour BP and rhythm monitoring and a pacemaker clinic. There is a Diabetes Day Resource Centre on site. The successful doctor will not have responsibility for any of these areas. WORK OF DEPARTMENT The Trust has decided to create two new Specialty Doctor posts in Tyrone County Hospital, Omagh to develop this stroke, orthogeriatric, palliative care and general rehabilitation service. This will bring the medical staffing complement up to 2 consultants and 3 specialty doctors. Support facilities include a full radiology department with spiral CT scanning, a dedicated day procedure unit and a modern laboratory. KEY DUTIES / RESPONSIBILITIES o The main focus of this post will be the clinical management of the inpatient rehabilitation and palliative care service. o The post holder will be a member of the Care of the Elderly department and have access to its secretarial service and provision of a personal computer. o The post holder will, in conjunction with the consultant organise the admission, management and care of patients admitted to the Care of the Elderly rehabilitation and Palliative care wards. o The post holder will participate in the teaching and training of medical students. o He/she will be expected to undertake continuing medical education (CME/CPD). o The post holder will participate in multi-professional audit and educational activities within the Unit and the Directorate. o The post holder may be required to deputise as far as is practicable for absent colleagues of the same or other grades not lower than the Specialty Doctor. o There are no over night on-call commitments with this post. o Any changes to the working arrangements will be by means of discussion and agreement (which should not unreasonably be withheld) with the post holder. SUPERVISION ARRANGEMENTS The post will be based in the southern sector of the Western Trust and in delivering services the post holder will: o Be responsible to the Director of the Primary Care and Older Persons Directorate. o Be clinically supervised by Dr E Hodkinson who will be on-site for 5 days per week and Dr F Robinson who will be on-site for 3 days per week. The consultant geriatricians in Enniskillen will provide supervision during annual leave periods. o Be accountable to the Medical Director for the maintenance of personal and professional standards. CONTINUING PROFESSIONAL DEVELOPMENT The Trust is committed to Continuing Medical Education. The medical library is supplied with books by the Medical Library of Queen’s University, Belfast and contains a personal computer with access to BMA Medline and the Cochrane Database installed on CD-ROM. Up-to-Date is networked on the Trust’s personal computers. Regular courses held locally include the annual Cardiology Update in the Erne Hospital, and an approved biannual ALS course alternating between the sites. There is a weekly radiology meeting telelinked from the Erne hospital. The unit takes part in both local and national audits e.g. Sentinel stroke audit The Trust supports the requirements for continuing medical education as laid down by the Royal College of Physicians and is committed to providing time and financial support for these activities. If it is not possible to agree a Job Plan, either initially or at an annual review, there are agreed procedures for facilitation and appeal with the final decision being binding on both the doctor and the Trust. TERMS AND CONDITIONS • The National Terms and Conditions of Service for Specialty Doctors will apply. • As per the current salary scale under National Terms and Conditions of Service for Hospital staff as updated via DHSSPS circular on rates of pay, the most recent HSS (TC8) 3/2008, with effect from 1 April 2008 to 31 March 2009 and all uplifts thereafter. • The full-time appointment will be for 10 PAs per week. • The successful applicant will normally be appointed to the grade for a one year probationary period which, if confirmed, will be extended without term and held until retirement under the terms and conditions of service. • The appointee will maintain registration with the General Medical Council (London). • Annual leave entitlement is 5 weeks (25 days) per year until 2 years service have been completed in the grade and 6 weeks (30 days) thereafter for practitioners remaining in the grade. Where the appointee in his/her immediately previous regular appointment attracted the 6 weeks annual leave per annum, this entitlement would remain. • Your private residence shall be maintained in contact with the public telephone service • He/she will be expected to be flexible and to co-operate with reasonable requests to cover for their colleagues’ absences where they are safe and competent and where it is practicable to do so. Where doctors undertake duties in accordance with this paragraph and such duties take place outside of their contracted hours, they will receive either an equivalent off duty period or remuneration. • The post will be subject to termination at any time, by three months notice in writing given on either side.
