									                    Instructions for completing Attachment A - HIV Performance Framework to Round 9 Proposal Form

Table 1 provides brief instructions for completing Attachment A to the Round 9 Proposal Form. Table 2 provides special instructions concerning Health System Strengthening
(HSS), Dual Track Financing (DTF) and Grant Consolidation. More detailed information on Performance Based Funding and Monitoring and Evaluation (M&E) can be found in
the Monitoring & Evaluation area of the Global Fund website available at: http://www.theglobalfund.org/en/me/

1. Goal(s):                           Goal(s) are broad and overarching statements of a desired, long-term outcome of the program and should be consistent with the national
                                      disease control strategic plan.
                                      Example : Reduce HIV related mortality in the country
2. Impact and Outcome indicators: Impact and outcome indicators related to the goal(s) should be included in this section. In the first column select whether this line will
                                  describe an outcome or an impact indicator. Under "indicator" a drop-down box with standard indicators, as specified in the Monitoring
                                  and Evaluation Toolkit , is provided. The Global Fund encourages the use of the proposed standard indicators; however, if appropriate for
                                  the local situation, one can overwrite this drop-down box to include country specific indicators. The selected indicators should be in line
                                  with the national disease control strategic plan and its respective M&E plan. For proposals or grants that include HSS interventions, HSS
                                  specific impact indicators should be selected from the drop down menu.
  Baselines:                      Baselines for impact and outcome indicators, if not available at the onset of the program, should be determined during Phase 1, i.e. the
                                  first two years of the grant. Under "Source" use the drop-down when appropriate or insert a new "Source". The "Source" should refer to a
                                  verifiable item, e.g. MICS, DHS. If the source of data to be collected is different from the source of the baseline (for more details, refer to
                                  the section on 'baselines' under item 6), please specify in the "Comments" field in the final column of the table. Also, any clarifications on
                                  "Sources" can be entered in the "Comments" field.
 Targets:                             Targets provided for impact and outcome indicators should include their frequency of measurement and should be provided only for the
                                      periods when data will be collected. For more details on the frequency of reporting for outcome and impact indicators see the Monitoring
                                      & Evaluation Toolkit p. 13, and in the corresponding disease specific sections p. 30 and p. 31.

3. Program Objectives:                Each goal should have a set of related, more specific objectives that will permit the Program to reach the stated goal(s). These objectives
                                      should be consistent with the objectives of the national disease control strategic plan.
4. Objective/Indicator numbers:       Indicators should be numbered according to the objective to which they relate, i.e. objective 1 indicator 1 = 1.1, objective 1 indicator 2 =
                                      1.2,…,objective 2 indicator 1 = 2.1,objective 2 indicator 2 = 2.2 etc.
5. Service Delivery Areas:            Under each objective, the key services to be delivered are defined under the respective Service Delivery Area (SDA). A drop-down box
                                      lists the Service Delivery Areas contained in the Monitoring and Evaluation Toolkit .
6. Indicators:                        It is recommended to refer to the list of indicators indicated in the Monitoring and Evaluation Toolkit . However, if these indicators do not
                                      adequately reflect the existing disease control program and strategies, other national indicators may be used. Please do not repeat the
                                      impact and outcome indicators in this section.
 Baselines:                           Baselines serve as the starting point for measuring the performance of the program. Baselines should take into consideration the latest
                                      available results of previous grants and/or the latest available information provided in WHO and UNAIDS reports and other valid sources.
                                      Baseline data (value, year and source) need to be provided for each indicator. In the "source" field a non-exhaustive choice of sources is
                                      provided in a drop-down box.


 Targets:                             Targets should be provided according to the frequency of measurement of a given indicator (please see Monitoring & Evaluation Toolkit,
                                      p. 13), if applicable, for every six months during year 1 and year 2 and for every year for years 3, 4 and 5. For targets expressed in %,
                                      clear numerators and denominators should be provided where relevant.
7. Directly tied / untied (shared):   Tick "Y" (yes) if program results are directly tied to Global Fund financing (activities are funded by the Global Fund solely). If targets
                                      described reflect results of a broader national, regional or institutional program to which Global Fund resources contribute and thus are
                                      shared targets between all donors involved, tick "N" (no). Indicators included in the same Performance Framework can be both, either
                                      "tied" (funded solely by Global Fund resources) or "untied" (shared funding by more than one set of resources), depending on the
                                      resources supporting achievement of related targets.
                                      For multiple grants per disease in a country: in case the multiple grants contribute to shared targets for the same indicators (and
                                      possibly other donors contribute to the given activity as well), the option "untied" should be chosen. In case the multiple grants have
                                      distinct targets for the same indicator and no other donor contributes to the given activity, the option "tied" should be chosen.
8. Baselines included in targets:   Baselines may or may not be included in the targets. They are not included if the Global Fund-supported intervention is not building on
                                    previous activities that were funded by other donors, but is starting new activities. Baselines are included in targets in cases where the
                                    Global Fund is supporting activities that have previously been funded by other donors and the Global Fund is ensuring continuity of
                                    funding. Applicants should indicate in the drop-down box whether baselines are included in the targets or not by ticking "Y" (yes) or "N"
                                    (no).

9. Cumulative targets:              Applicants should clearly specify in the drop-down box whether targets are: a) cumulative over program term, i.e. target is maintained or
                                    increases across program term (in that case each target is equal or greater than the previous period's target), b) cumulative annually, i.e.
                                    target is maintained or increases across one year of the program starting counting from zero each year, or c) not cumulative, i.e. only
                                    current period target is put and is independent across periods.
                                    Example: Suppose there are 500 health centers nation-wide and you propose to increase the number of health centers with consistent
                                    supplies of ARV drugs from a starting point (baseline) of 100 and increments of 100 additional heath centers supplied each year. You can
                                    record this according to the following options:

                                    a) "cumulative over the program term" (baseline not included in targets)

                                    Most important, regardless of the option chosen, targets need to be self-explanatory. Please tick in the box
                                    option a), b) or c) and if, necessary, specify further in the "comments" field in the final column of the table.


10. DTF: Name of PR responsible     When a holistic Performance Framework is prepared for the proposal submission, list in this field the PR(s) who will be responsible for the

                                        Special Instructions related to HSS, DTF and Grant Consolidation
1. Health Systems Strengthening:    A) When HSS interventions included in the proposal are specific to a particular disease component, Applicants should include the
                                    relevant indicators in the diseases specific Performance Framework either under HSS specific SDAs or other SDAs.
                                    B) When HSS interventions included in the proposal are cross-cutting and benefit more than one disease, HSS specific SDAs and
                                    indicators should be incorporated as a separate objective in one of the selected disease specific Performance Frameworks.

2. Dual Track Financing (DTF):      Proposals including two or more Principal Recipients (PRs) for the same disease component should submit one consolidated Attachment
                                    A (including all PRs). This consolidated Attachment should clearly indicate who the implementing PR is for each target, in column Q. In
                                    cases where two or more PRs are contributing to the same indicator, Attachment A should list only one consolidated target (i.e., the sum
                                    of each PR contribution), but should indicate the names of each contributing PR in column Q. In addition to the consolidated Attachment
                                    A, one Attachment should be submitted separately for each implementing PR. In exceptional cases when the planned activities of the
                                    different implementing PRs are not related to each other, separate Attachments may be submitted without consolidated Attachment A.
3. Grant consolidation:   Applicants willing to consolidate their R9 proposal with a previous grant for the same disease component with the same PR need to
                          ensure that the objectives, service delivery areas and indicators of the R9 proposal are aligned with those of previous grant(s). Note that
                          grant consolidation will only occur following Round 9 proposal approval.
