         Entry will ONLY be accepted upon receipt of Direct Deposit slip from Bank along with your
                                             REGISTRATION FORM received by the above deadline date.

            PRIZES & Trophies (may change depending on committee &
                            number of teams entered)

                                                                                               PROCEDURES
1. ONLY teams officially entered by the deadline will be accepted.
2. ROSTERS will be completed and submitted by each team before their first game.
3. Rules, regulations and tournament schedule to be picked up at Soccer Tent.
4. Official schedule will be posted in the Registration Tent, please check schedule for
         accuracy.
5. Maximum number of players for each team: a) 18 for men; b) 15 for women; c) 20 for
         youth; d) 14 for 10 & U and 7 & U.
6. ID required for proof of age and Indian ancestry.
7. Player can only be registered on one team and one division only.
8. NO DRUGS OR ALCOHOL PERMITTED
9. Environmental Awareness (B.Y.O. Cups and Utensils)
10.Teams will be contacted by phone.
            TO AVOID DISAPPOINTMENT, PLEASE REGISTER WITH PAYMENT
