The Maryland Developmental Disabilities Administration and the Maryland Developmental
Disabilities Council are pleased to announce the following Request for Proposals:

# 09-QA-01 Money Follows the Person Family-to-Family Mentoring for
Supporters of Individuals with Developmental Disabilities Residing in State
Residential Centers

The Maryland Developmental Disabilities Administration (DDA), in partnership with the
Maryland Developmental Disabilities Council, is inviting proposals that will develop and
demonstrate an effective, meaningful family mentoring program for the family members and
supporters of individuals transitioning out of State Residential Centers to living in the
community. Family mentoring programs should include, among other things, outreach to
families, and the opportunity to be matched with a mentor, support, and educational resources
that will increase knowledge of home and community-based support services and the advantages
associated with community living for persons with developmental disabilities. Up to $87,804 is
available in Fiscal Year 2009.

A pre-proposal conference will be held on Oct. 1 at 2:00 p.m. at the Oakland Mills
Meeting House, Columbia, Maryland. For directions, go to the Council’s website or call
the Council office at 410/767-3670, ext. #1 or toll free at 1-800/305-6441. The pre-proposal
conference will be the only opportunity to ask questions. Attendance, however, is not mandatory.
If special accommodations are needed, please contact the Council office by Sept. 25.

Proposals are due to the Council by October 16, 2008, no later than 4:30 p.m.
