Family Information Forms

Your Family Information Form will be sent home with our second September newsletter. However, it is critical that we
know right away about any special medical conditions such as bee or peanut allergies or asthma so that our staff can be
prepared in the event that something happens. If it is necessary for your child to have medication at school or if you need
to leave an Epipen at school for allergy purposes, Board policy requires that you and your doctor complete and submit the
appropriate medical/consent forms to the school. These are available at the office and we would be happy to assist you in
completing them.

PLEASE NOTE: Our Lady of Lourdes Catholic School is a NUT-FREE school.

We would also ask that you indicate on the form if your child is to do anything other than take the bus or walk home at
the end of the day. For example, if your child is to walk to a babysitters or is to be picked up by someone at the end of
the day on a regular basis, please indicate this on the form. In the interest of reducing the possibility of confusion and the
risk of sending your child to the wrong place, any change to this regular arrangement must be made in advance in
writing. We are always putting the safety of your children first and appreciate your co-operation in this matter!

Similarly, you are reminded that our Board’s Safe Schools Policy requires all visitors to the school to check in with Mrs.
Burns at the office to sign in and obtain a Visitor’s Pass. This allows us to keep track of who is in the school at any given
time and provides additional security for our students. This ensures that classes are not interrupted and enables teachers
to continue to provide quality instruction to students.

H1N1

Please find attached a letter from the KFL&A Health Unit regarding H1N1. We will continue to update you with
information as if becomes available.

School Agendas

Once again, the school has purchased Student Agendas for students in Grades 1 – 8.

Agendas help students to organize their time and set priorities. Students develop good work habits and study skills when
they use their agendas to record upcoming tests or assignments, homework due dates as well as extra-curricular activities
and sports events. A consistent study routine/schedule at home each day and faithful use of the agenda by parents as well
as students can make a great difference in student achievement! The agenda is also an excellent tool for teachers to
communicate with parents and for parents to keep in touch with what is happening in their child’s class. For our younger
students, you will find important newsletters and notices in the plastic pouch at the front. We will also use the agendas to
record student lates each day.

Because it is vital for every student to have an agenda, we are asking for $5.00 for each agenda from parents (to a
maximum of $10.00 per family). We would appreciate it if you could forward payment to your child’s teacher as soon as
possible and don’t forget to look for and initial the agenda each night.

Advance Notice - School Open House and Professional Development Day

Our school Open House will take place in the evening on Thursday, September 24 from 7:00 until 8:00 p.m. This is a
good opportunity to come out and meet old friends and our staff members. The following day, Friday, September 25th,
is a Professional Development Day.

We look forward to having a very successful and enjoyable school year.

Landscaping

We are so delighted with the wonderful, professional landscaping at the front of the school. I extend my appreciation to
Mr. Mark Wensing and his company for donating shrubs, plants, soil and many hours of manual labour in creating such
beautiful gardens. Thank you very much.

Youngest and Only

We have decided to change the way our information is sent home. Instead of sending it to the oldest student in the family
we will now send it with the youngest.

Beginning with our October newsletter we will only be sending it home with families that have requested a “hard copy”.
Otherwise it will be posted on our website for you to download at home.

To access our electronic newsletter

Open your Internet browser. Type in http://schools.alcdsb.on.ca/lady/
On the right hand side of the page you will see “Newsletter”. Click on the newsletter you wish to read.

We will continue to send home the monthly calendar for you to post.

Yours very truly,

Mrs. Judy Hay
