					                    WLUSA - Instructions for using Shift Premium Form

A form for reporting WLUSA shift premiums is now available on the WLU website
under Human Resources, then Payroll. This form has been designed to allow you to
report shift premiums to payroll in one of two ways:

   1) you can fill in the form for each day of any pay period in which shift premiums
      are owing (14 rows have been provided).

   For example, for the pay period January 1, 2006 to January 14, 2006, you can advise
   that on Thursday, January 4, 2006 three (3) hours of shift premiums should be paid
   and on Thursday, January 12, 2006 three (3) hours of shift premiums should be paid.
   No other shift premiums are anticipated at this time.

   You would complete the form as follows:

Shift Premium (daily hours worked outside 8:30 am-4:30 pm)
  From Date:        To Date:      Hours to be Paid Premium                 Comments
                                  (please note if this is hours
                                     per day or per week)
 January 4, 2006     January 4, 2006      3 hours / day
 January 12, 2006    January 12, 2006     3 hours / day



   2) you can fill in the form instructing payroll to pay a defined number of shift
      premium hours over a span of time.

   For example, for the period January 1, 2006 to April 30, 2006, pay this employee
   three (3) hours of shift premium every week.

   You would complete the form as follows:

   Please note:
   In order to ensure that an employee is not overpaid, please report shift premium
   payments under either option 1 or option 2 but do NOT complete both options for the
   same employee. For example, if you complete and submit the form instructing
   payroll to pay Sally three (3) hours of shift premiums every week from January 1,
   2006 to April 30, 2006, do NOT also complete and submit a form bi-weekly for the
   same hours.
