					  Patients’ experience of direct referral from Optometrists cataract pathway
The Royal Sussex Eye Hospital in partnership with local optometrists, and the Primary Care Trust have
recently introduced a new referral and treatment process for cataract surgery. To help use improve the system
and ensure that it meets the needs of all patients it is important to find out what your experiences was like
We would be very grateful if you could complete this questionnaire and return it to the optometrist’s
reception or in the envelope provided. You have our assurance that any information given will be treated in
complete confidence and will not influence your treatment in any way.

                                             About your visit to the optometrist
This first set of questions are about how long you waited to see the optometrists and what happened during
your appointment and examination.

 1.      Did you see your GP before seeing your optometrist about your eye problem?
