Modern Apprenticeship Centre (MAC) Approval
To offer the Customer Service Modern Apprenticeship in Scotland you must first register your
Organisation with the Institute of Customer Service.

The registration process involves the following six steps.

Step 1    Complete the attached MAC Approval Form and send it to the ICS with associated
          evidence.

Step 2    On receipt and approval of this application the ICS will email your confirmation of registration
          with:

   A letter of approval
   A New Modern Apprentice Registration Form
   A New Certificate Application form

Step 3    You must complete a Modern Apprentice Registration Form for every Apprentice you
          wish to register, within 8 weeks of starting their apprenticeship. This should be sent to the
          ICS with a cheque for £50, made out to the Institute of Customer Service. This amount
          covers the cost of both Modern Apprentice Registration and Certification.

Step 4    Confirmation of the Apprentice Registration will then be sent to you with the Name and
          Apprentice Serial Number listed, you will need to keep the serial number on file.

Step 5    When an Modern Apprentice has completed the full Framework you will need to send a
          completed Certificate Application Form for the apprentice with associated evidence.

Step 6    A Modern Apprenticeship Completion Certificate will then be produced and forwarded to
          you.

may feel it is necessary.

A Personal Development and Training Plan
A Training Agreement signed by the Apprentice
An Induction Programme
An ongoing review of the PDTP

I declare that this organisation will ensure that all apprentices have achieved the mandatory outcomes, as
specified in the Framework, before application for certification is made.

Two units from another single S/NVQ
The five Core Skills – separately certificated
Customer Service Level 3 S/NVQ

I understand that:

   the cost of registration for individual candidates is £50 payable on submission of an Apprentice
    Registration Form and is inclusive of the cost of the final certificate
   the ICS has the right to withdraw registration should any of the declarations shown above be in question
