                                 Required Items for CCB Year-4 Annual Progress Report
                               Reporting Period: May ’07 - Apr ’08 (Since last year’s progress report)

        Please email all materials by Mon. Apr. 28, 2008, according to this schema:

        To write & submit your report, please coordinate with all members of your group and your group Leader.

1. Report Summary (for Standard NIH Report): Approximately 2-page progress report summary for each
   core or project (See page 2).

2. Narrative (for Specialized Report): A narrative section describing the progress in each of the seven cores
   over the past year.

3. Highlights: A brief description of any highlights (less than a year old, which acknowledges support from
   the CCB). Highlights should be based on a publication or other product of the NIH. The highlights should
   be written at a level that is understandable by a technically literate, but non-expert individual. Include any
   Press and Media coverage!

4. Publications: Report those publications (papers/book chapters/abstracts) that are published, in press,
   and submitted resulting directly from this grant (during the reporting period of May ’07 - Apr ’08). The
   following two lists of publications should be presented separately.

               A list of publications that acknowledge support from the NIH grant U54 RR021813.

               A list of publications that used the Center but did not acknowledge support.

    List the complete citation of all publications, provide a link to a publicly available on-line journal or the NIH
    PubMed identification number (PMID), see examples below. You may search a PMID for your published
    paper from this website http://www.ncbi.nlm.nih.gov/sites/entrez?db=pubmed. Please send us a pdf file or
    a hard copy of the publication if an on-line link or PMC number is not yet available.

6. UCLA Investigators: If there are any personnel changes in your group, please provide new employees’
   names, role on project (i.e., PGR, GSR, Lab Assistant, etc), effort %, and base salary. Indicate who are
   being replaced when applicable. Inform Sandy Lui with any budgetary changes that you would like to
   make for the forthcoming budget period (8/1/08-7/31/09).

7. Subcontract Sites: Sandy Lui will contact each subcontract site regarding budget and administrative
   related pages separately.

    **As a reminder, please fully spend your year-4 funds by 7/31/08, no carryover is allowed**
