         The Stipulation and Agreement of Settlement (“Settlement Agreement”) dated August 8, 2006, provides benefits to
all persons who entered into a contract for a Debt Management Plan with CCCC from November 27, 1996, through
September 18, 2006, and who have made a valid claim for such benefits.

          I am hereby making a claim for a share of the amounts paid by CCCC pursuant to the Settlement Agreement
pending approval of the Settlement Agreement by the District Court. I understand that the Settlement Agreement is a partial
settlement because it resolves all claims the class members have or may have against CCCC, but that any claims against
other defendants may continue.
