Notes for Suppliers
The purpose of this questionnaire is to assist The Forestry Commission in deciding
which suppliers to short-list to invite to tender for Waste Management services on
Forestry Commission Sites in the Thames Chase Community Forest and Jeskyns
Community Woodland.

Returned pre-qualification questionnaires will be scored against the criteria set in the
‘Scoring Methodology & Weighting’ document. Only the top five scoring contractors
for each contract will be invited to the full tender process.

In order to simplify this process, you don't need to provide supporting documents, for
example, accounts, certificates, statements or policies with this questionnaire, unless
requested. However, the Forestry Commission may ask to see these documents at a
later stage. You may also be asked to clarify your answers or provide more details.

Please answer every question. If the question does not apply to you please write N/A
(not applicable); if you don't know the answer please write N/K (not known).

"Authority" means the purchasing organisation that is seeking to award a contract.

The Forestry Commission will be awarding two contracts to suppliers who will be able
to provide the specific services of the contract.

We anticipate that both contracts will start on 1st April 2010 for 3 years.

Contract 1: Thames Chase waste management contract covering 10 community
woodlands in South-East Essex.

Contract 2: Jeskyns waste management contract covering 1 community woodland in
North Kent (just outside Gravesend).

 A timetable highlighting key phases of the anticipated tendering processes is detailed
below and should provide bidders with an overview of dates for involvement of key
personnel. Please note the dates are provided for guidance purposes only and may be
subject to change.

Contract Type & Term of Agreement
The Forestry Commission (FC) will be awarding each contract as a direct award,
awarded to one supplier, who is able to provide ongoing waste management services
to 10 sites in the Thames Chase Community Forest and to 1 Community Woodland in
North Kent.

It is anticipated that the Contract will commence on 1st April 2010 for a period of three
years (3 years).

Scope of Supply

The Forestry Commission is looking for expressions of interest on two contracts:

Contract 1:

The Forestry Commission own and manage 10 community woodlands within the
Thames Chase Community Forest (South-East Essex). These sites sit predominately
within the London Borough of Havering with the exception of one which sits within
Thurrock. Each site has a network of hard paths and infrastructure and range in size
from 12ha to 74ha. All the sites are either ex-Greenfield or ex-Brownfield and have
been developed since 2001 to provide public greenspace for local people.

The successful contractor will be required to undertake a range of operations across
all sites, these shall include:

   Empty of dog waste bins (once a week)
   Weekly litter pick of 7 car parks
   Litter pick off 10 sites (every 6 weeks)
   Removal of fly tipping

Contract 2:

Jeskyns Community Woodland is located approximately 1km southeast of Gravesend
(Kent) and is a 147 hectare greenspace opened in 2007. The site was formally a
typical arable farm that has been developed into a community woodland consisting of
areas of woodland planting, open grassland/meadows and orchards.
The successful contractor will be required to undertake a range of operations across
all sites, these shall include:

   Empty of dog waste bins (Twice a week)
   Litter pick of car park (Twice a week)
   Litter pick off site (As requested)
   Removal of fly tipping
   Manual or mechanical clean of car park surface
   Supply and install of dog waste bins
   Removal and disposal of dog waste bins

Notes:

"System" means processes and procedures to ensure that the subject is properly
managed. This includes making sure that legal requirements are met.


Health and Safety Policies
Any business employing five or more people has, by law, to prepare and bring to the
attention of employees a written Health and Safety Policy Statement.

A Health and Safety Policy usually consists of three distinct sections namely:

General Policy Statement - a short statement outlining the organisation's commitment
to Health and Safety signed and dated by the senior organisation official (for example,
the Managing Director).

Organisation - how the organisation addresses health and safety; lines of
