Gill Managed Care Initiative CAP (1) Assist geriatric training for MedPartners using practice enabling strategies (2) TCGEC will assess and tailor content of programs to the needs of providers (3) Use of EPOMS to track results  IV) V) Texas Woman's University – Susan Wittjen Geroethics Education for Graduate Students and Health Professionals in Geriatrics CAP (1) Integration of geroethics content into 8 courses (2) design of CME courses for professionals (3) Students are agreeing to a 3 yr. follow-up study  University of North Texas – Ann Reban Enhancing Case Management through CQI Education for Adult Day Care services professional staff and HMO staff CAP (1) One pilot center will track care outcomes and compare with standards and finally, communicate results (2) EPOM will examine extent to which the Adult Day Care Centers employ CQI to improve programs  UT Houston – Ellen Stevens Enhancing the Knowledge and Skills of Paraprofessional Service Providers Working in Elder Care CAP (1) Assessment of needs, measurement of present knowledge & attitudes, measurement of clients needs and on-site training (2) Content: psychosocial assessment, dementia, depression, Rx side effects, advanced directives (3) EPOMS will measure knowledge and skills  Texas Southern University, Houston – Maxine Hammonds-Smith Family Management System Project CAP (1) Equip family care givers with case management skills to complement paraprofessionals (2) Partnership with churches families and home health agency staff (3) EPOMS will measure familial capabilities  Texas A&M University – Judy Warren Rural Interdisciplinary Outreach for Elders & Paraprofessionals, Paraprofessionals and Family Care Givers CAP (1) Work with selected counties to further develop county agent's capabilities to arrange for CE programs (2) Elder Care Conferences in rural counties (3) Adult Family Care conference  UT Houston HSC – Sharon Ostwald Improving Quality of Long-term Care in Community and Institutional Settings CAP (1) 5 CAP teams (comprised of staff from nursing facility or community agency in Houston/Galveston area and at least one nursing faculty member) (2) Center on Aging will develop clinical outcomes measures Subcontract for CAPs – Jeffrey P.
