Form Reference: Issue Level: Form Name: Retention: File Location: PU005GF 28 – September 2004 Supplier Profile Questionnaire 2 Year after Document Completion Date Purchase Department (TKA Use Only) Rationale System Purpose The purpose of the Supplier Profile Questionnaire is to allow suppliers within a common commodity to be rated against each other giving TKA a valuable tool for the nomination of suppliers for future business. Supplier Ratings Once completed suppliers return questionnaires to the Purchasing Department who then mark them in line with the rational attached and allocate an overall mark, which follows the traffic light system. The traffic light system is as follows: Green There are no concerns with the supplier’s ability to supply TKA and can/will be used for current and future business. The supplier’s results show slight concerns, which require further investigation. However the supplier is still eligible to be nominated for current and future business. The suppliers’ score is un-acceptable to TKA and the supplier is listed as a non-preferred supplier, who should only be used if circumstances are beyond TKA’s control to dictate (e.g. customer nomination). Amber Red The rating given to suppliers depends on the total score the supplier achieves for their responses detailed in the supplier profile questionnaires, the total marks scored will ensure suppliers are categorised as follows:Category Green Amber Red Score 500– 750 350 – 499 Less than 349 14 Notes   The maximum marks available to a supplier are 750. Attention is to be given to sections 1.2, 2.2 and 2.3. Should the supplier not score in any of these three sections then the maximum overall rating the supplier could achieve would be amber, irrespective of score. Further investigation must take place prior to order consideration. If supplier is not ISO9000 accredited, automatic RED will be allocated. Exceptions will be made in cases where suppliers score less than 400 points, if they can show a good past performance record in the areas of Quality, Cost and Delivery, they may be considered for future business nominations or qualify as a development source. Exception will also be made when a customer has nominated a supplier and Tallent has had no option but to use them.
