15 Years of Caring for Gaston County
September 30th marked the completion of Gaston Family Health Services’
2007 fiscal year, and our fifteenth year of providing family medicine
services for the under-served residents of Gaston County. Thanks to the
Director’s Letter
wisdom and continuing support of the GFHS Board of Directors, Gaston
County Government, Caromont Health, the North Carolina Office of
Research, Demonstrations and Rural Health Development, and count-
less other advocates, we have been able to serve more than 45,000
citizens since our inception. We have grown in response to the needs of
our patients from a small, crowded clinic in the Health Department to a
full-service primary medical and dental program providing over 40,000
annual patient encounters to more than 20,000 citizens annually (1 in 9
Gaston County residents). Our primary health care services are designed
to encourage preventive care as well as providing care to the sick. This
not only saves the community dollars, but also helps keep children healthy
enough to learn and adults healthy enough to work.
GFHS must maintain financial viability. We must provide care for our
patients that meets and exceeds the community standard for private
practices. The Medicaid patient of previous years, who had few choices
for care, is now competed for among private providers. We must be the
provider “of choice” for these patients. Maintaining this patient base,
a population with choices, is extraordinarily important to our ability to
provide for the community’s underserved. Revenues from Medicaid and
Medicare patients help underwrite the costs of providing services to our
largest and neediest patient group, the uninsured.
GFHS continues to explore new revenue sources and the development
of service lines that address the needs of our patients. In 2002, GFHS
applied for, and received, designation as a Federally Qualified Health
Center. In FY 2007, under a special High Poverty New Access Point
grant opportunity, GFHS was asked to expand its services to Iredell
County. Once again, GFHS applied for, and received funding. With
support from many stakeholders in the Iredell community, operations at
Statesville Family Medicine and the Open Door Dentistry began in
December 2007. We look forward to continued collaborations with
Iredell County for this important community initiative.

Dibert. A nurse case manager provides additional help to senior patients
by arranging their refferals, connecting them to community resources,
advocating in their best interests and helping them to navigate through a
difficult healthcare system.
Dr. Stitt came to GFMC after serving as Gaston Memorial’s vice presi-
dent of medical affairs. A family practitioner with a geriatrics back-
ground, he wanted to return to work that made an immediate
impact with patients. And at 61, he could empathize with many con-
cerns about aging. “Plus,” Dr. Stitt says, “I think old folks are plain old
fascinating”. He says that he has never felt more appreciated from the
way that his grateful patients make him feel by bringing him cornbread,
tomatoes, a birthday cake and an afghan. Dr. Dibert, an internal medi-
cine physician, was added to further compliment services at Gaston
Family Medical Center.
Gaston Family Medical Center offers free health risk assessments for new
senior patients as well as primary care services for both well and sick
care, chronic disease management for conditions that affect seniors most
such as diabetes, heart disease, COPD and arthritis. Ancilliary services
provided for seniors include diabetes education and management,
nutrition counseling and a clinical social worker. There is an after hours
nurse advice line for those patients who need questions answered after
the Center is closed. A consulting pharmacist is also available to help
seniors negotiate the Medicare Part D maze.
The senior-friendly design of the Center includes a covered drop-off
area, convenient parking and wheelchair access. The carpeted exam
rooms are all located on one level. Exam rooms have comfortable chairs
instead of flat tables, and the waiting room television plays nature videos
instead of nerve-jarring news updates.
Gaston Family Medical Center is open Monday - Friday 8am to 5pm.
l Initiated and expanded a centralized Call Center to schedule patient appointments for
Gaston Family Health Services, Senior Care and Workplace Wellness patients.
l Developed a New Patient Information Brochure to better inform users of the Gaston Family
Health Services’ system.
l Received High-Poverty New Access Point grant funding to expand medical, dental, and
behavioral health services at two locations in Iredell County.
Accomplishments
l Received Expanded Medical Capacity grant funding to expand evening and weekend hours.
l Partnered with Gaston Memorial Hospital and many local providers to develop the Garibaldi
Run. The race raised $25,000 to use towards needed specialty healthcare services for
uninsured patients of GFHS.
l Expanded Workplace Wellness activities to Pharr Yarns, Inc. and the Gaston County
Government as well as added additional on-site clinic hours at Affinia/Wix.
l Performed Health Risk Assessments to 500 Stabilus, Inc. employees and nearly 1,500
Gaston County Government employees, spouses, and retirees.
l Opened Gaston Family Medical Center to Workplace Wellness patients.
l Participated in the UNC-Chapel Hill Management Academy for Public Health with four
Gaston County Health Department employees to develop a business plan for the Highland
Health Center.
l Recognized as one of Gaston County’s Public Health Heroes for work to protect the health
of Gaston County residents.
l Began renovations of the Women and Children’s Counseling Center.
l Contracted with Cabarrus Family Medicine, a federally-qualified health center located in
Concord, NC, to provide management services on an interim basis.
l Initiated a medical and dependent flexible spending account for employees.
l Expanded the Medication Assistance Program to Cherryville, NC in collaboration with
Cherryville Area Ministries.
l Participated in a review by the National Health Services Corp and received a
favorable report.
l Contracted and enrolled in all NC Medicare Part D participating plans.
l Added Human Resources and Information Technology staff to support organization activities.
l Hired a new Dental Director, Dr. William Donigan.
l Collaborated with Community Health Partners to hire a Licensed Clinical Social Worker
to provide behavioral health counseling to Medicaid children in Gaston County.
l Coordinated with the Gaston County Health Department to provide behavioral health
counseling services to women and children.
l Launched GFHS website, www.gfhs.info.
l Initiated marketing campaign to promote pediatric dental services at the GFHS Dental Clinic
and the Pediatric Dentistry, as well, as Senior Care services at Gaston Family Medical Center.
l Hired a dental access coordinator to develop and initiate a school group dental program for
the Gaston County Schools. Transportation to the dental clinic for students was provided by the
school system.

l The overall GFHS payor mix for FY 2007 indicated that 51% were self-pay,
20% Medicaid, 15% Medicare, and 14% private insurance.
Statistical Highlights
l Gaston Diabetes Center for FY 2007 had 826 patients participate in diabetes group
or individual counseling education opportunities; 130 uninsured diabetes patients
received opthamological screenings for diabetic retinopathy and other eye diseases;
and 186 uninsured diabetes patients participated in on-site podiatry clinics. In
FY 2007, GFHS served 5,408 patients living with diabetes through 11,573
encounters.
l The Mediation Assistance Program (MAP) helped 278 new and returning patients in
FY 2007, generating 5,328 prescriptions valued at more than $2.1 million.
l In FY 2007 the Workplace Wellness Program patient encounters amounted to 3,480
at the on-site healthcare facilities and 516 off-site at Gaston Family Medical Center.
l The Behavioral Health Counseling Program treated 681 patients through 3,089 visits
in FY 2007 for depression, anxiety, substance abuse, psychosis, etc.
l There were 195 HIV/AIDS patients in FY 2007 who generated 738 encounters at
GFHS. 120 (62%) received additional case management services. 36% of GFHS’
HIV patients are women.
l Community Health Partners (CHP) was composed of 43 primary care practices
serving 24,410 Carolina Access enrollees in FY 2007; 19,883 in Gaston County
and 4,527 in Lincoln County. As members of the CHP network, GFHS and Bessemer
City Health Care Center served an active caseload of 2,079.
l The Health Check Coordination program participation ratio for Gaston County
improved from 66.1 in FY 2005 to 69.6 in FY 2007. Data for FY 2008 supports a
continued increase.
l Through the School Group Dental Program, the GFHS Dental Clinic served 329
students from 4 elementary schools, 23 students from the 21 Century Afterschool
program and 79 students from the Headstart program.

The grant funds a Case Manager, reimburses GFHS for the cost of care,
and pays for patient referrals to specialty physicians.

The Gaston Family Health Services Board of Directors is composed of community members and patient

the majority of the GFHS Board members are consumers of GFHS health care services.
