Completion & Submission of Electronic Copy of Application Go to the DPI website: http://dpi.wi.gov/fns/cacfpapps.html. Follow the steps below to properly submit an electronic version of your FY 2009 Application to your DPI Consultant. (1) Once you have pulled the application up on the website, save it to your computer in a file that you will be able to find it in. (2) Complete the application by filling in the relevant boxes and fields. (3) Save your completed copy on your computer. (4) You may print your completed copy to keep on file at your site; however, DPI will send you a hard copy of the approved application. You may either mail a paper copy or email an electronic version of the application. When mailing a paper copy, the authorized representative must sign page 13 of the application renewal and the back page of the site application (if applicable). Mail 2 copies of the completed application. -OR- E-mail your assigned Consultant: (1) The completed application as an attachment to the email, (2) Copy and paste the Email Certification Statement (enclosed) into the body of the email. (3) Include the name of the Authorized Representative, agency name, agreement number, address and phone number in the body of the email. (4) In the subject line of the email, include “Application” along with your agency’s agreement number. If you have any questions, contact your assigned Consultant.
