RMKS/1. THIS IS A COORDINATED COMMANDER, U.S. FLEET FORCES/COMMANDER, U.S. PACIFIC
FLEET MESSAGE. CANCEL REF D.
2. PMV MISHAPS ACCOUNT FOR THE LARGEST NUMBER OF OFF-DUTY NAVY FATALITIES EACH
YEAR. NON-FATAL, OFF-DUTY PMV MISHAPS ALSO RESULT IN A SIGNIFICANT NUMBER OF LOST
WORK DAYS. MANY OF THE MISHAPS ARE AVOIDABLE, AND WE WILL WORK DILIGENTLY TO
IDENTIFY CAUSAL FACTORS THAT WE MIGHT BE ABLE TO INFLUENCE OR CONTROL IN ORDER
TO REDUCE THESE TYPES OF MISHAPS.
3. PER REF A, EFFECTIVE IMMEDIATELY, ALL COMMANDS WILL INVESTIGATE ALL OFF DUTY PMV
MISHAPS THAT RESULT IN, OR MAY POTENTIALLY RESULT IN, A PMV CLASS A OR B
MISHAP. CLASS A PMV MISHAPS ARE DEFINED AS MISHAPS RESULTING IN A FATALITY OR
PERMANENT TOTAL DISABILITY; CLASS B PMV MISHAPS ARE DEFINED AS MISHAPS RESULTING
IN A PERMANENT PARTIAL DISABILITY OR THE INPATIENT HOSPITALIZATION OF THREE OR MORE
PERSONNEL. COMMANDS WILL COMPLETE THE PMV MISHAP INVESTIGATION
REPORT (PMV-MIR) TEMPLATE TO DOCUMENT ROOT CAUSES AND CORRECTIVE ACTIONS TO BE
IMPLEMENTED. THE PMV-MIR TEMPLATE, AS WELL AS THE PROTOCOL FOR CONDUCTING THE
INVESTIGATION AND THE ENDORSEMENT PROCESS, ARE LOCATED ON THE NAVAL SAFETY
CENTER WEBSITE (HTTP:/WWW.SAFETYCENTER.NAVY.MIL/PMV/DEFAULT.HTM).
4. COMMANDS UNDER COMUSFLTFORCOM/COMPACFLT ADCON WILL ENDORSE THE CLASS A
PMV INVESTIGATION REPORT THROUGH THEIR FIRST FLAG OFFICER IN THE CHAIN OF
COMMAND. THE ECH 3 COMMANDER WILL PROVIDE FINAL CHAIN OF COMMAND ENDORSEMENT
TO COMMANDER NAVAL SAFETY CENTER WITH COPY INCLUDING ALL ENCLOSURES TO FFC N4S
AND COMPACFLT N01CE2. DUE TO REVISION ON ENDORSEMENT PROCESS, CANCEL REF D.
5. THIS INVESTIGATION AND ENDORSEMENT PROCESS REPLACES THE REPORTING OF CLASS A
PMV MISHAPS UNDER THE FIRST FLAG NOTIFICATION PROGRAM PRESCRIBED IN REF B. ALL
OTHER FIRST FLAG REPORTING REQUIREMENTS AND ENTRY OF INFORMATION IN THE WEB
ENABLED SAFETY SYSTEM REMAINS IN EFFECT. THIS DOES NOT CHANGE THE REF B
REQUIREMENT FOR INITIAL NOTIFICATION OF MISHAP OCCURRENCE OR REQUIREMENTS FOR
CONVENING THE APPROPRIATE INVESTIGATION AND MAKING THE REQUIRED LINE OF DUTY
DETERMINATIONS PER CHAPTER 2 OF REF C.
6. THIS NEW INVESTIGATION AND REPORTING PROCESS SERVES TO ENHANCE
EXISTING REPORTING REQUIREMENTS AND WILL AID IN PRESERVING OUR MOST VALUED ASSET,
OUR PERSONNEL.
