           CAMBRIDGE COMPETITIVE MUSIC FESTIVAL 2010




 Please complete the form in CAPITAL LETTERS and in black ink
 For duet/trio classes, enter the names of both/all competitors as ONE ENTRY
 Enter competitor’s age at 1st January 2010 if under 18 years.
 Tick the box if the Official Accompanist is required (an additional fee of £15 will be payable)
 Fees, which will be collected in January, are as follows:
        o Soloist                             -       £8 per entry
        o Duet/Trio/Quartet                   -       £4 per performer
        o Ensemble/Choir                      -       £16 per entry
 DO NOT send entry fees with the entry form. These will be collected in January when you
    receive the timetable.
 Details of Own Choice music will be required at time of payment
 Entries must reach the Festival Administrator by Friday 4th December 2009.

 Please enclose an A5-sized SAE which will be used to send class times etc in January.
