									                         Membership Application Instructions:
                             1. Complete all sections on the Phi Theta Kappa application. Please
                                make sure that your address, telephone, and email are current.
                             2. If you are an Associated Students member, include your 4 digit A.S.
                                number. Note: you do not have to be an A.S. member to apply for
                                Phi Theta Kappa membership.
                             3. A check or Money Order (please keep your stub!) payable to
                                Phi Theta Kappa for:
                                $75 (Full Membership) OR $25 (Provisional Membership).
                             4. Put the application and payment in a sealed envelope; drop it off in
                                the “Student Drop Box” in the Mailroom (A-8) to the attention of Lan
                                Hong by 4pm, April 30th, 2010.

What Happens Next:                For Full members, after verification of your G.P.A, your application will be
submitted to Phi Theta Kappa International Headquarters.
   1. You will receive membership confirmation e-mail.              This email contains your membership
        identification number allowing you full access to exclusive membership benefits.
   2. A member benefit packet will arrive via mail (usually within 8-weeks) containing your Golden Key
        Membership Pin and detailed information on securing your exclusive membership benefits.
   3. Please attend the New Member Orientation scheduled for Wednesday February 10th at 10 a.m.
        and Friday February 19th at 1:00 p.m. in Room A-15a
   4. You will be invited to attend the Phi Theta Kappa New Member Induction Ceremony on Saturday
               th
        May 8 , 2010 at 1:00 pm in Rm. D-121 (faculty lounge).
Types of Membership:
    1. Full Membership ($75) - Full members are recognized on the Campus, Regional, and International
       levels with Phi Theta Kappa. They can attend and participate in meetings, attend Phi Theta Kappa
       Conferences and Conventions, vote for officers and on issues, and run for Local, Regional, or
       International Offices. Only Full Members are eligible for the Phi Theta Kappa scholarships valued
       at over $36 million annually. Full Members also receive the Golden Key Membership Pin, a
       Membership Certificate and a Membership Card issued by Phi Theta Kappa International,
       membership notation on official City College Transcripts and on your Certificate/Degree at the time
       of graduation, and access to the Members-only section of www.ptk.org. To be a Full Member you
       must be enrolled at San Diego City College, have completed at least 12 units, and have a
       Cumulative GPA of 3.50 or higher for the past 5 years.

    2. Provisional Membership ($25) – Provisional Members are recognized on the Campus level only at
        San Diego City College and are not officially recognized by Phi Theta Kappa International. They
        can attend and participate in meetings, attend Phi Theta Kappa Conferences and Conventions, and
        vote for officers and on issues. Provisional members will receive a Phi Theta Kappa Provisional
        Membership Certificate, issued by the Beta Iota Kappa Chapter. Provisional members will not be
        entered into the international registry and will not receive a Phi Theta Kappa notation on
        transcript/certificate/degree. To be a Provisional Member you must be enrolled at San Diego City
        College and have a Cumulative GPA of 3.00 or higher.
     **EOPS students: Check with EOPS office to see if your membership fee is reimbursable.

Win Full Membership (valued at $75)!
Your local Phi Theta Kappa chapter will grant two free Full Memberships for eligible students in the
Spring 2010 semester. This is an essay contest, consisting of 200 words or more entitled, “What Joining
Phi Theta Kappa Means to Me”. All submissions are due via E-mail to the President, Liam Liedorff, at

Membership must be PAID IN FULL. Essay winners will be reimbursed after selection process.

  Please Note: For Phi Theta Kappa you need at least a cumulative GPA of 3.0 or higher to qualify as a
        Provisional member, and a cumulative GPA of 3.5 or higher to qualify as a Full member.

chapter. I believe in and support the purpose of the Society as stated in the Phi Theta Kappa Constitution, adhere to the moral standards of the
Society, and currently enjoy the full rights of citizenship, freedom, and privileges of my country (International students attending college on a
United States student visa and who possess full rights of citizenship of their country are eligible for membership). I understand that there is a
required GPA for maintaining membership in Phi Theta Kappa and agree to notify my chapter advisor immediately if at any time I fail to
maintain my chapter’s maintenance GPA. Therefore, I solemnly promise to uphold the standards of Phi Theta Kappa, and to keep this object and
aim in mind, and I do solemnly pledge allegiance to my fellow members and promise to aid them in all worthy endeavors.
