Colorado Voter Registration Instructions
Source: Colorado Office of the Secretary of State When to Register to Vote In Colorado, you must register to vote 29 days before an election. This means you must be registered no later than 4:00 p.m. on October 4, 2004 in order to vote in the November 2, 2004 elections. When to Use the Voter Registration Card If you are a Colorado citizen who lives or has an address in Colorado, you can use the voter registration card to: • • • • Register to vote in Colorado Report a change of name on your voter registration Report a change of address on your voter registration Choose a political party or change parties
Who May Register to Vote To register to vote in Colorado, you must be: • • • • • A citizen of the United States A resident of the Colorado town or city from in which you desire to vote At your present address at least 30 days before an election At least 18 years old by Election Day Able to attest to the oath on the registration form
Please note the following exceptions: • • • No person while serving a sentence of detention or confinement in a correctional facility, jail or other location or while serving a sentence of parole shall be eligible to register to vote or vote in an election. A confined prisoner who is awaiting trial but has not been tried shall be certified by the institutional administrator and shall be permitted to register to vote by mail. Individuals serving a sentence of probation may register but may not vote in any election.
How Often You Must Register A registered voter who fails to vote in a general election shall have their registration record marked "Inactive” by the county clerk and recorder following the general election. The United States postal service will deliver “inactive” voters a registration card. If the county clerk and recorder receive no response to the confirmation card for two general elections, the county clerk and recorder shall cancel the registration of the voter.

How to Register You may register to vote by completing a Colorado voter registration card. Registration is available at many places in Colorado, including: • • • • Most state agency offices Most public libraries At any county clerk and recorder's office Through the Internet -- www.sos.state.co.us/pubs/elections/main.htm
Registration may also be available at other state service agencies. Contact the county clerk and recorder's office near you to find out which service agencies provide voter registration. You may also register by mail. How do I determine if I am registered to vote? To determine if you are registered, call your local county clerk and recorder. Contact information listed at the end of this information packet I think I registered to vote a long time ago at a different address, but I’m not sure where. What should I do? Contact your Secretary of State’s office or fill out a voter registration card with your current address. Put the address where you think you were last registered in the previous address box. How do I know if my county clerk and recorder received my registration card? To know if your county clerk and recorder received your registration card, call you county clerk and recorder (see information attached at the end of this packet). Hearing impaired people call the State TDD number 1303-869-4867. How to fill out the Voter Registration Form Section A. Box 1 • Full Name Put your full name in this box in this order Last, First, Middle. Do not use nicknames. Note: If this application is for a change of name, on the bottom half of the form, please give your full name before you changed it. Box 2 • Home Address (where you currently live) Put your home address in this box (legal address). Do NOT put your mailing address here if it is different from your home address. Do NOT use a post office box or rural route without a box number. Note: If you were registered before but this is the first time you are registering from the address in Box 2, in Section B, Box 2 (on the bottom half of the form) please give the address where you were registered before. Please give us as much of the address as you can remember. Box 3 • Mailing Address If you get your mail at an address that is different from the address in Box 2, put your mailing address in this box. Box 4 • Date of Birth Put your date of birth in this box - in this order - Month, Day, Year. Be careful not to use today’s date! Box 5 • Gender Select your gender from the available choices. It is a requirement to vote. Box 6 • Drivers License Number or Dept of Revenue ID number Put your Drivers License number in this box. It is a requirement to vote.

Box 7 • Social Security Number Put your Social Security Number in this box. Including at least the last four digits is a requirement to vote. Box 8 • Telephone Number Most states ask for your telephone number in case there are questions about your application. However, you do not have to fill in this box. Box 9 • Choice of Party You must register with a party if you want to take part in that party’s primary election. If you want to register with a party, chose the party of your choice. If you do not want to register with a party, check “Unaffiliated.” Box 10 • Signature Review the information in Item 7. Before you sign or make your mark, make sure that: • • You meet Colorado’s requirements and You understand that you are signing under the penalties of perjury if the form is signed and you know the information to be false.
Finally, sign your full name or make your mark, and print the date in this order - Month, Day, Year. Note: First Time Voters Who Register By Mail If you are registering to vote for the first time in Colorado and are mailing a registration application, a copy (do not include original documents) of one of the following is required with your mail-in registration form • • • • • • • A valid Colorado driver’s license A valid Colorado Department of Revenue identification card A valid U.S. passport; a valid pilot’s license with photograph issued by the Federal Aviation Administration A valid employee identification with a photograph issued by the U.S. Government, Colorado state government, or any country municipality board, authority, or other political subdivision of the state A valid U.S. Military Identification card with photograph A copy of a current utility bill, bank statement, government check, paycheck, or other governmental document that shows your name and address At least the last four digits of your social security number.
If you do not submit proof of identification with your mail-in registration form, you will be required to provide proof of identification using the types of identification described above. How to Submit Your Application Give your application to your Democracy Leader or Voter Volunteer, or mail your application to your county clerk and recorder’s office no later than 29 days before the election. Colorado Voting Instructions
Am I eligible to vote? You are eligible to vote if you are: • • • • A citizen of the United States At least 18 years old by Election Day A resident of the city or town from which you desire to vote for at least 30 days Registered to vote at least 30 days prior to the election
Where do I vote? Call your County Clerk and Recorder (Specific contact information can be found at the end of this information packet), watch for listings in the local newspaper, visit the Secretary of State’s Web site at http://www.sos.state.co.us/ and choose Election 2004, or call the Elections Division of the Secretary of State at (303) 894-2200. What is Early Voting and how can I do it? In Colorado, there is a period of time before the election when you can vote in person at places designated by your County Clerk's office. Early voting starts 15 days before each General or Coordinated election, and 10 days before each Primary election, and ends several days before the election. If early voting is not possible for you, you can vote by Absentee Ballot.
Voting by Absentee Ballot
Am I eligible to vote by absentee ballot? In Colorado, any registered voter may apply for an absentee ballot. There is no need to provide a reason. Voters may vote absentee by mail or in person. How do I apply for an absentee ballot? You may obtain an application for a mail ballot from your County Clerk and Recorder. A request for a mailed absentee ballot must be received in writing at the County Clerk's Office no later than the close of business on the Tuesday before the election. When can I apply for an emergency absentee ballot? Emergency absentee ballots are available only within 20 days of the election. Emergency Absentee Ballot requests must be made before 5:00 PM and returned no later than 7:00 PM on Election Day

Am I eligible for an emergency absentee ballot? You are eligible for an emergency absentee ballot if: (1) You are qualified and a registered voter; and (2) Circumstances arise within 20 days of an election that would make you eligible to vote by absentee ballot. How do I get an emergency absentee ballot? An application for an emergency absentee ballot can be obtained from your local board of canvassers. For the November 2, 2004 general election, the application must be delivered to the local board of canvassers no later than 4:00 p.m. on the day before the election. If you are applying for an emergency mail ballot on medical grounds, you must include a certificate from a physician or Christian Science practitioner attesting that the illness or disability did not manifest itself until 20 days or less prior to the date of the election. Emergency absentee ballots may be voted at the local board, mailed to you, delivered to a person presenting written authorization from you, or delivered by a pair of supervisors depending on the circumstances of the application. Your emergency absentee ballot must be received at the location established by the County Clerk no later than 7:00 p.m. on Election Day. How do I identify myself at the polls? When you enter your polling place, state your name and address to one of the supervisors sitting at the checkin table. He/She will announce your name and residence and give you a ballot application to fill out and sign. Your name will be located and checked off on the voting list, and you will be allowed to vote. If you reside at an address that is different than what they have on the voting list, or if you are listed as inactive, you will be required to fill out and sign an affirmation form before voting. If you believe you are registered but your name is not on the list, you may still vote by provisional ballot as regulated by The Help America Vote Act. A provisional ballot will allow you to vote at your polling place, and have election officials’ check your registration status later. How do I vote for a write-in candidate? In an election, if the name of the candidate you wish to vote for does not appear on the ballot, you must print the name of the person on the blank line labeled write-in AND you must complete the arrow pointing to the person’s name. In order for your vote to be counted, the candidate must have filed an affidavit of intent with the designated election official. Can I cast a straight party vote? There is not a way to vote for all candidates for a specific political party. You must vote for each political race separately. What happens if I make a mistake when marking my ballot? If you make a mistake, do not erase or cross out on the ballot. Bring the ballot to the clerk. In order to protect the secrecy of your intended vote, the clerk will ask you to complete several additional arrows on the ballot. The clerk will then issue a new ballot. What happens if I deposit my ballot into the voting equipment and I have voted for too many candidates? Voters who select multiple candidates for a single office will be alerted to the consequences of such action before their ballot is submitted. Technology will allow voters to make a selection and then change it, if they so desire, before casting their vote.

Is there someone to help me if I have any questions on marking my ballot? The clerk who presents you with your ballot, may, if requested, explain how to mark the ballot. The cover of the secrecy sleeve and the inside wall of the voting booth will also contain instructions on how to mark a ballot. You may also take this booklet or any other materials into the booth to assist you in voting. If desired, you will also be able to request the assistance of a bi-partisan pair of supervisors. If qualified, you may also bring a person of your choice into the voting booth with you. The warden at the polling place will have the necessary affidavit for such voters. Are there special accommodations for voters who are blind or visually impaired? Colorado law provides that a voter has a legal right to assistance in voting if assistance is needed because of blindness or other physical disability or inability to read or write. The following procedures apply: • • • The voter must inform one of the election judges that he or she needs assistance. The voter may be assisted by any election judge or by any eligible elector selected by the voter. The person selected must complete a ‘voter assistance/disabled voter self-affirmation form
The person selected may not be an election judge, spouse, parent, grandparent, sibling, or child, eighteen years of age or older, of the voter requesting assistance. The selected individual may provide the voter any assistance needed, including entering the voting booth and preparing the ballot or operating the voting machine. What can I do if my polling place is not accessible to the handicapped? According to law, whenever practicable, polling places shall be situated in buildings that are architecturally and structurally suited for ready access by the handicapped. If you are handicapped and the polling location you are assigned is inaccessible, you can notify the local board of canvassers at least 5 days in advance of Election Day and they will arrange an alternate means to allow you to cast your vote. If the polling place does not offer access for a physically disabled voter, an election judge shall assist the elector in casting a ballot outside the polling place within 100 feet of the building.
