Enclosed you will find an Affidavit of Registration, an Affidavit Questionnaire and an Out of the
CNMI Voter’s Individual Information form. These forms are necessary for us to determine your
eligibility to vote in the Commonwealth elections and to process your registration card once your
eligibility has been determined and your registration is approved.

Please complete all the non-shaded portion of the Affidavit of Registration and have it notarized
by a Notary Public (for military personnel, a commissioned officer may sign in lieu of a notary
public). Also, please ensure that you complete the Affidavit of Questionnaire and the Out of the
CNMI Voter’s Individual Information form. Lastly, please ensure that you attach either a copy
of a valid United States Passport, naturalization documents or a Certificate of Live Birth to
the Affidavit of Registration and return it to us as soon as possible.

If you will be out of the Commonwealth during the election, you will need to fill out an
Application for Absentee Voting. Please keep in mind that we are not authorized to accept your
Application for Absentee Voting earlier than 75 days and/or not later than 25 days before the
election. This form is necessary in order for your absentee ballot to be sent to you; otherwise
you will have to vote personally at the polls here in the CNMI on Election Day.

Instructions to completing affidavit of registration.


1 CMC §§ 6205 (c) (1) to (4) require that you provide us with your social security number1 , your full legal name, your date and place of birth and your residence in the
Northern Mariana Islands, including a mailing address in the Northern Mariana Islands.

For village, please include the village of your residence in the Northern Mariana Islands. Island should also include the island of residence in the Northern Mariana
Islands. Those who reside in Oleai Village should place Oleai under Village and Saipan under island. Those who reside in San Jose Village should place Tinian as
their island of residence.

If you have registered before, and have since had a change of name, due to marriage, divorce, or had a legally approved change of name, please register under your
present name but include your previous name in the line provided in the affidavit of registration. Please include a legal document authenticating the change of name.

If you are changing your election district, complete your registration as you normally would except that you will indicate your new village (and island, if necessary) and
we will make the change of election district.

Those who are registering in the northern islands should place “Northern Islands” under both village and island.

All registrants who have not previously provided the Commission with evidence of their U.S. citizenship or national status are required to do so. You need only provide
the Commission with this evidence once. Evidence of U.S. citizenship status accepted are naturalization certificate, citizenship certificate, current U.S. Passport, or a
certificate of live birth issued by a government. Those born outside of the United States or its jurisdictions must provide the Commission with other documents that
show that they qualify for U.S. Citizenship. Former TTPI citizens who were in the Northern Mariana Islands from November 4, 1981 to November 4, 1986 may be
required to provide evidence of continuous domiciliary and residency in the Northern Mariana Islands for five years (1981 to 1986) as required by Covenant Article
3(b).

The affidavit of registration must be notarized and must be mailed directly to the Commonwealth Election Commission, P. O. Box 500470, Saipan, Mp, 96950-2924.
Facsimile or other electronic forms will not be accepted. The law requires that an envelope must be postmarked from a location outside of the Northern Mariana Islands
for registration for those residing outside of the Northern Mariana Islands.

If you are registered to vote in a jurisdiction outside of the Northern Mariana Islands, you must re-establish your domiciliary and residency in the Northern Mariana
Islands 120-days before the date of the election.

§ 6201. Voting: Eligibility.
(a) Pursuant to Article VII, Section 1 of the Constitution of the Commonwealth of the Northern Mariana Islands, a person is eligible to vote who, on the date of the
election, is 18 years of age or older, is domiciled in the Commonwealth, is a resident in the Commonwealth and has resided in the
Commonwealth for a period of time provided by law, is not serving a sentence for a felony, has not been declared by a court to be of unsound mind, and is either a
citizen or national of the United States.
(b) Pursuant to Article VII, Section 2 of the Commonwealth Constitution, a person shall not be denied the right to vote because that person is unable to read or write.
(c) Under authority of Article VII, Section 1 of the Commonwealth Constitution, no person shall be eligible to vote who has not resided in the
Commonwealth at least 120 days prior to the election day
(d) Consistent with Article VII, Section 1 of the Commonwealth Constitution, no person who is confined to a mental institution shall be eligible to vote.
(e) In accordance with Article VII, Section 1 of the Commonwealth Constitution, no person serving a sentence for a felony, as defined by Section 6003(t) of this part,
shall be eligible to vote.
(f) No person shall register or vote in any other precinct than that in which the person resides.



Instructions to completing questionnaires.

The questionnaires are for the Commission’s use to determine your eligibility to register to vote in the Northern Mariana Islands. The first questionnaire is necessary.
You must mark “No” to all three boxes to be eligible to vote in the Northern Mariana Islands. You must also be away from the Northern Mariana Islands for such
purposes as education, employment, medical, business, etc. but with the intention that the Northern Mariana Islands is your domiciliary and your residence outside of
the Northern Mariana Islands, no matter the length of time, is temporary. On this questionnaire, you should include your residence where your presently reside.

The second questionnaire helps us to communicate with you. It allows us to get in touch with you through the telephone, email or mail. Provide as much information as
you could. None of the information on the second questionnaire will be used to determine your eligibility to register to vote in the Northern Mariana Islands.



Instructions to completing questionnaires.

The application to vote absentee should be completed only if you are registering to vote or if you are already a registered voter. Only registered voters could be issued
an absentee ballot. Please ensure that the address you provide in this application is the address where you want your ballot mailed to. Once a ballot is issued in your
name, you will not be issued another ballot nor will you be allowed to vote at the polling place on election day. Again, ensure that the address you place on the ballot is
the address where you will be in September or October of the election year.

Finally, please ensure that your signature on your affidavit of registration, your questionnaire, your application to vote absentee, and your affidavit of absentee voting
are the same. The Commission, by law, is required to compare the signatures to ensure that they match. Signatures that do not match may be rejected and the ballots
are either not issued or the ballots are not counted.
