MISCELLANEOUS INFORMATION
TIMELINES OPV (live vaccine) was given PRIOR to 2000. Select OPV if it was given in 1999 or earlier, unless specifically identified in the shot record. IPV (inactivated vaccine) was given AFTER 1999. Select IPV if it was given in 2000 or later, unless specifically identified in the shot record. MMR, Varicella and/or Influenza Nasal Spray (and any other “live” vaccine) must be given on the SAME DAY or 28 DAYS APART or more. Not doing so will cause the vaccines to be ineffectual.
Identifying the appropriate polio vaccine will help prevent invalid immunizations given, such as MMR and/or Varicella.
DTP was given PRIOR to 2000. Select DTP if it was given in 1999 or earlier, unless specifically identified in the shot record. The 4th DTaP must be given no earlier than 6 MONTHS from the 3rd DTaP, no matter when the 3rd DTaP was given. The Pediatric Hep A series is a two dose series. The Pediatric Hep B series is a three dose series. 4
