        MetLife Auto and Home Insurance Program

As of January 1, 2006 Marsh@WorkSolutions has assumed enrollment and billing responsibilities for all
MetLife automobile, homeowner and renter’s insurance policies offered to Banner Health employees.

Q: Why Marsh?
A: Marsh@WorkSolutions PersonalPlans is an organization that specializes in the behind-the-scenes
administration of voluntary benefits such as the MetLife Auto and Home Insurance Program.
PersonalPlans provides enhanced services including online auto/home quotes, responsive customer
service and payment options that include automatic payroll deduction and individual premium payment
arrangements. Best of all, because PersonalPlans specializes in voluntary benefit programs beyond the
MetLife discount insurance program, this change will allow Banner the opportunity to offer you
additional voluntary benefit programs in the future.

Q: I currently take advantage of the MetLife automatic payroll deduction program. How does this
affect me?
A: The transfer of management requires no action from you. Except for the administration, this change
will not affect your current MetLife coverages or premiums. Payments can continue to be made via
payroll deduction. Previously, your MetLife payroll deduction was labeled as “MetLife” on your pay
stub. At the beginning of 2006, this changed to “Personal Plan.”

Q: Has the Auto, Home & Renter’s discount program through MetLife changed?
A: No, the discount program will remain the same. The program will continue to be offered and
underwritten by MetLife and there will be no change to your coverage choices and premiums. The only
difference is that now you will call the PersonalPlans Service Center when you have questions about the
program or if you are interested in receiving a free, no-obligation quote on coverage.

Q: What do I do if I have a claim?
A: Call 800-854-6011 as soon as possible to discuss your coverage. For auto accidents, it is important to
let the insurance company know about the incident even if it appears minor and regardless of who is at
fault. The Claims phone number is also listed in your policy information.

Q: How do I get a quote?
A: Getting a free, no-obligation quote is easy. Depending on the state in which you live, your options may
be different from those listed below. Generally, you can:

        Call the Service Center at 800-906-8793 to work with an insurance counselor over the phone.

        Complete the online quote process by visiting www.Personal-Plans.com/bannerhealth. Simply
        select Get a Quote and Enroll and answer a series of questions regarding your insurance
        requirements and driving history. The site will guide you through the process with helpful tips
        and easy to understand explanations. Based on the information you provide, you will receive an
        immediate quote. In some more complicated cases, you may be instructed to contact the Service
        Center to speak with an insurance counselor.

        For the most accurate comparison, have your current insurance policies on hand. For an auto
        quote, also have your Vehicle Identification Number (VIN) of each car you’d like to insure as
        well as the driver’s license number for each driver in your household.

Q: When can I enroll?
A: You can apply for coverage anytime. There are no enrollment dates.

Q: Who can apply?
A: You are eligible to apply for the MetLife discount insurance program if you are scheduled to work 32
hours or more a pay period and are classified as a regular full-time or part-time employee. Also, you must
have a valid U.S. driver’s license. You may also apply for insurance for dependent children who reside in
your household.

Q: Why should I buy my auto, home or renter’s insurance through PersonalPlans?
A: Banner Health is providing access to the discount insurance provided through PersonalPlans as part of
your benefits program. For this reason, there are several advantages to buying insurance through
PersonalPlans:

        Special program rates that are often lower than individual rates

        Convenient payroll deduction of your premiums

        Special discounts including a payroll deduction discount.

Overall, this program provides high-quality insurance at competitive rates to most people. Because auto,
home and renter’s insurance is based on several factors including the state you live in, your driving
history, your vehicle, who’s insured, and many other factors, this insurance may not give you the lowest
rate, but don’t you owe it to yourself and your hard-earned money to find out?

Q: When would coverage begin?
A: In most cases your insurance can start the day after you accept the coverage over the phone. It can also
begin at a later date designated by you. It’s up to you. Just speak with the insurance counselor to decide
when you want your coverage to begin. If you want your new coverage or a change in coverage to take
place sooner rather than later, PersonalPlans can fax you temporary insurance cards. Your permanent
cards will be mailed directly to you.

A: By far, the most convenient and cost-effective method of payment through this program is payroll
deduction. PersonalPlans can work with you on alternative methods of payment, but you might lose some
discounts if you choose methods other than payroll deduction.

A: For more information about the auto, home & renter’s discount insurance program, go to

an insurance counselor. For claims, call 800-854-6011. The Service Center is available to take your call
Monday through Friday between the hours of 7:00am – 5:00pm (Central Time).
