TRAINING OBJECTIVES
1 Describe the treatment planning process, elements and criteria as a roadmap to recovery and wellness. Identify how emerging new frameworks of person-centered recovery, resiliency and wellness differ from past and current practice to guide the plan of care. Utilize a strengths based person-centered and culturally competent assessment and formulation as the basis for the treatment/recovery plan roadmap. Assist individuals/families to articulate and prioritize person-centered goals and discharge/transition needs. Partner with the client/family to develop and prioritize objectives, interventions and services that reflect client/family strengths, preferences, choices and stage of change. Facilitate implementation of the plan including timely review and update of the assessment and plan as necessary. Define key elements of documentation (including Medical Necessity) and properly document the elements of the plan and services provided.

Feel free to duplicate this form for additional registrations. Breakout sessions will be divided into adult or child/ adolescent population groups. Payment: A check or money order made payable to the Fairfield County ADAMH Board must accompany the registration form. Credit cards cannot be accepted and invoices will not be mailed. TRAINING OVERVIEW
This one day workshop will help direct care staff, clinical supervisors, administrators, and reviewers to examine current practice, the shift towards a recovery/resiliency and person-centered framework and how to better use the individual service/recovery/ wellness/resiliency plan as a roadmap to effective rehabilitative services and supports rather than being an administrative requirement. It is not uncommon to find that despite a strong commitment to the values of recovery/resiliency and wellness, current practice in service planning and delivery remains embedded in a provider centered service system. Participants will discover how helping consumers and families to create a meaningful plan can become a vital clinical tool that better reflects consumers' preferences and choices while promoting recovery/resiliency and wellness in their mental health and addiction treatment. With lecture and discussion to set the stage, the main focus of the workshop is the opportunity to actually experience writing plans using case studies to build skills in developing and writing person-centered goals, objectives and therapeutic interventions. Breakout sessions will be divided into adult or child/adolescent populations. The presenters will provide specific, individual coaching to improve the ability of participants to develop a treatment plan that guides a process of recovery/resiliency and healing. Due to the intensive, hands-on nature of this workshop, sessions are limited to 60 participants each.

PRESENTERS
Neal Adams, MD, MPH, is currently Director of Special Projects at the California Institute for Mental Health and is the former Medical Director for the California Department of Mental Health. Dr. Adams is a board certified psychiatrist and addictions specialist who has recently served as the President of the American College of Mental Health Administration. Diane Grieder, M.Ed., has more than 20 years of experience in the behavioral healthcare field, devoted to quality improvement functions and management systems. She has been an accreditation surveyor of behavioral health programs with CARF for the past 16 years. She conducts training nationally on clinical documentation and treatment planning, performance improvement, and strategic planning. Dr. Adams and Ms. Grieder are coauthors of a newly published book, Treatment Planning for Person-Centered Care, The Road to Mental Health and Addiction Recovery, which each participant will receive as part of the course materials.
