Instructions for the Preparation of Manuscript
Copy should be t y p e d double-spaced on one side only of 8~/~ x 11 paper. E v e n m a t e r i a l t h a t is to be set in a t y p e size smaller t h a n t h a t of t h e main text (footnotes, references, etc.) should be double-spaced. T h e a u t h o r should supply a short title for use as a r u n n i n g head. This should be t y p e d on a separate sheet as t h e first page of m a n u s c r i p t . Type t h e complete title of the article on a second sheet, a n d the a u t h o r s ' names a n d affiliations on a t h i r d sheet. All m a t e r i a l t h a t is to be set in a t y p e size different from t h a t of t h e m a i n t e x t should be t y p e d on separate sheets. This includes t a b u l a r m a t t e r , case histories, quotations, formulas a n d other subsidiary m a t t e r in t h e text, footnotes, bibliographies, a n d legends for illustrations. Displayed formulas which are to be built up to more t h a n one line of type, even if t h e y are t h e same size t y p e as t h e text, should be on a s e p a r a t e page. Legends should not be a t t a c h e d to or w r i t t e n on t h e i l l u s t r a t i o n copy. Positions for t a b l e s a n d figures in t h e text m a y be i n d i c a t e d in t h e m a r g i n of the text page. M a n u s c r i p t s t h a t do n o t conform to t h e s e i n s t r u c t i o n s will be cut a n d r e a r r a n g e d so t h a t t h e m a t t e r to be set in different sizes of t y p e can be separated.
Galley proof. A u t h o r s will receive galley proofs of t h e i r papers set separately as outlined above. Galleys ~'ill be a r r a n g e d in the following order: tables, r u n n i n g heads, footnotes, figure legends, text, bibliography. All m a t e r i a l t h a t has been cut from t h e body of t h e text will be i n s e r t e d in its proper place in the galleys. This includes formulas, quotations, case histories, a n d other subsidiary m a t t e r . M a n u s c r i p t will be arranged in this order: formulas, tables, footnotes, figure legends, t e x t inserts including bibliography, text. Illustrations. Copy for illustrations should be u n m o u n t e d glossy p h o t o g r a p h s clearly identified as to number, a u t h o r ' s name, and t o p of figure in soft crayon on the back. Graphs should be in I n d i a ink on bluelined paper. If larger t h a n 9 x 12", t h e y m u s t be accompanied b y glossy p h o t o g r a p h s within t h i s size.
