1.1 System requirements for METS users
**Please note, METS has certain minimum system setting requirements.
Microsoft Internet Explorer 6 and 7 only. Versions supported are:
• Internet Explorer 6 on Windows XP Professional Service Pack 2
• Internet Explorer 7 on Windows XP Professional Service Pack 2
IE browser must be set to:
• Enable pop-ups.
• Enable cookies.
The MFDA METS Administrator will provide each Member Administrator their METS Login and initial password. It is the
responsibility of each Member Administrator to set up Member user accounts.
Information for METS Administrators appears in section 11.

1.2 Logging in to METS
1. In your Internet browser, type www.mfda.ca/mets or go to www.mfda.ca and click on the METS link.
2. The METS Login screen opens.
3. Type in your Login and Password. Remember that your password is case sensitive.
4. Click Login.
Note: For security purposes, you should log into METS manage users to change your Administrator appointed password.
See Changing your password below
If your Account Status is “Inactive” then you will be prevented from continuing. Contact your Member Administrator to
activate your user account.

1.3 Changing your Password
If you are just logging into METS for the first time or if you have forgotten your password and a new one has been sent to you, for
security reasons you should change your password.
To change your password, you must first login to METS using the ID and Password that was provided to you by the Member
Administrator.
1. Click the Change Password button on the Login screen.
2. Type in your Login and current password. Enter the new password in the New Password dialog box and the Verify New
Password dialog box.
3. Click Save.
If you fail to properly confirm you new password you will receive an error. Try again.
If you are successful, you will see this message and be returned to the Login screen so that you can login with the new password.

4. Type in your Login and Password and click Login.
1.4 Forgotten Password
Use the forgotten password feature to reset a forgotten password. This feature generates a new password and emails the new
password to your the email address as known by METS. To use, select the Forgot Password? link on the METS login screen.
From the Reset Password form, enter your Login Name and click on the Reset Password button. A new email address will be
emailed to you shortly.
1.5 Welcome Screen
The Welcome screen provides a brief description of the METS system as well as links to other important information.

Near the bottom of the Welcome screen, you will see a list of any Concluded events associated with your Member that are not yet
Locked as well as a list of any Pending Customer Complaints.
You can view the Event Report Form for any event listed on the Welcome screen by clicking on the METS reference number.
2 NAVIGATING THE METS SYSTEM
The functional components of METS use common elements. Once these are understood, you will be able to quickly navigate through
the system to find information you are seeking.

The left side of the METS window
provides links to functions within the

If one of the links, such as Create an
Event appears greyed out it means that it
is not applicable or not available to your

• Français toggles the language from English to French
• Help opens up the METS Software User Manual
• Contact Us provides contact information for METS Support
• Logout logs you out of the system

3 CREATE AN EVENT
To make a report under MFDA policy 6 parts A and B, the Member must create an event on METS. The Create An Event function
allows you launch the Event Report form. This form appears in sections to permit related data to be entered to summarize the nature
of the event and how the Member has responded to it.
The sections, which are common to all Event Types, are described below to assist you in completing the Event Report.
To start:
1. Ensure that you are logged into METS.
2. Ensure that your Browser is set to allow Pop-ups for the METS site (how to disable pop-up blocker)
3. Click Create An Event on the left-hand side of the screen.
4. Select the Mandatory event criteria:
a. Event type
b. Date of initiation
c. Subject type

3.1 Event Type
Select the appropriate Event Type by clicking on the Radio Button to the immediate left of the name of the appropriate event type.
The event types are:
1. Bankruptcy
2. Civil Claim
3. Criminal Charge
4. Customer Complaint
5. Denial, Cancellation, Suspension or Termination of Registration or Approval
6. External Disciplinary Action
7. Garnishment
8. Other Theft, Fraud and Serious misconduct
9. Termination, Compensation Paid to Client and Internal Discipline (not otherwise reported above)

3.2 Date of initiation
Once you have selected the event type, a corresponding message appears below the Date of initiation field. These messages are a
reminder to select the appropriate date for each event type.
Event Type Date of Initiation Message
Bankruptcy The date of bankruptcy filing
Civil Claim The date that the civil claim was filed
Criminal Charge The date the criminal charge was laid
Customer Complaint The date the complaint was received by the Member firm
Denial, Cancellation, Suspension or Termination of Registration or Approval The date of the denial of application or registration
External Disciplinary Action The date the subject was named as a defendant
Garnishment The date of garnishment
Other Theft, Fraud and Serious misconduct The date you became aware of the serious event
Termination, Compensation Paid to Client and Internal Discipline (not otherwise The date the disciplinary action was taken or serious event
reported above) occurred
The date of initiation defaults to the current date but you must set it as appropriate to the event.
3.3 Subject Type
The Subject Type is the role of the subject against which the event is being filed. Your choices are Member or Individual.
3.3.1 Subject Type is Member
Select this subject type for only those events that do not involve individuals.
When you select Member as the subject type, the Event Report Form will open. The subject-Member relationship section of the
event report form will not be editable as it is not applicable with this option.

3.3.2 Subject Type is Individual
When you select Individual as the subject type you are forwarded to the Subject Search screen in order to find and select the
Individual to whom the Event pertains.
3.3.3 Subject Search
The search fields use the criteria you enter as follows:
• In any part of First, Last or Middle Name: Will return individuals if text anywhere, in any name field, matches what you have
entered. For example, if you enter “Jo” in this field, individuals “John Smith”, “Mary Smith-Jones”, and “Betty-Jo Miller” would all
be returned in the search results.
• Last Name, First Name, or Middle Name: Will return individuals if the name starts with the character or characters you
entered. For example, if you enter “ST” in the first name field, individuals STeven Carver and STephanie Cooper would be
returned in the results, however, JuSTin Maguire would not.
• NRD Number and Province of Residence Will return individuals only if an exact match exists to the values you entered.
NOTE: If you click the search button without entering any search criteria, all individuals entered by your Member will be included in
your search results.

Once you have entered your search criteria, click Search. You will be presented with a list of results matching your query.
If the desired individual is found, click on the Radio Button next to the name to select it. Click next and the Event Report Form will
open in a new window
If the desired individual is not found, but you are certain the individual has been added, click the Back to Search button to return to
the Subject Search Screen where you can attempt another search using broader search criteria. (Use jo, instead of Joseph, for
example.)
If the Subject is not in METS, you can click the Add New Subject radio button and click Next.

3.3.4 Add New Subject
If the individual has not been entered previously for your Member, you will have to add them to METS by clicking the Add New
Subject radio button, which will bring up the new subject fields.
To add a new individual for your Member (including former employees / agents) complete all of the fields shown below.
All fields are Mandatory if known. However you may not have all data available when creating the event and, in such cases, data
can be omitted. However, first and last name fields (marked with an * ) must be completed to allow you to proceed.
When you are done entering individual subject information, click Next and you will launch the event report form that you can now
begin to edit the event form.
Pop Up Reminder – When you click Next to create an event, if you have not enabled pop-ups, you will not see your new event, but
it has been created. Before proceeding, disable pop-up blocker, and search for the event you just created and edit as normal

3.4 Special notes to creating an event
Event Type and Subject Type are mandatory fields. You will receive an error message at the bottom of your window if you try to
continue without making these selections.
Choosing the Other Theft, Fraud and Serious misconduct and Termination, Compensation paid to Client and Internal Discipline
event type will bring up the following message:
If you select Yes:
You will be forwarded to the Search An Event screen so that you can find and update the existing event.
This feature helps prevent you from adding a duplicate Event Report for a single event.
If you select No:
You will proceed with new event creation as normal.

4 EDIT AN EVENT
You can open an event report form to update it in two ways: By creating a new event; or by searching for a previously entered event
and clicking on the METS Reference Number.
4.1 Event Report Form Tips:
• In the Event Report Form, when you see an item preceded by a plus sign it means that the item can be expanded to show more
information. Conversely, a minus sign it means the item can be collapsed to hide additional information.
Collapsed Expanded
• In the Event Report Form (and anywhere in METS) Fields marked with an Asterisk * are mandatory.
• In any field involving a Dollar amount (settlements, compensation, or commissions withheld, for example) round to the next
whole dollar. So $1349.34 becomes $1350.

4.2 Open Event Report form
As long as an Event is not locked, most information in the Event Report Form can be edited.
Note: You can only edit one section of the Event Report Form at a time. If you have clicked the Edit button of a particular section,
you must click the Save or Cancel button in that section before you can edit another section.
4.3 Subject-Member Relationship
The Subject-Member Relationship is a description of the Subject’s relationship to the Member at the time the event occurred.
Click the Edit button, and then select the current relationship to the individual from the three options in the drop down menu
• Current employee/agent, event occurred here
• Current employee/agent, event occurred at past employer
• Former employee/agent, event occurred here
Note: This section is not editable if you selected Member at event creation.

4.4 Branch
Note: The Branch is the registered location where the subject was working at the time the event allegedly occurred. Member head
office addresses are included in METS and are the default location.
To change the Branch:
1. Click Edit to enable editing the section
2. Previously entered addresses appear in the drop down menu. Click the arrow of the drop-down list and select the
appropriate branch. If you cannot locate the Branch from the list of choices, you can click the Add button. You will be
required to fill in Branch details.
As with all areas of METS, all fields are required, if known. Fields marked with a red asterisk * are mandatory

4.5 Nature and Scope of Allegation
Depending on the Event Type you choose, different information related to the event type selected will be required. For example,
if External Disciplinary Action was selected, you will be required to indicate the Regulatory Body imposing the discipline.
The list of Violation Types is also dependent on the Event Type you have selected, and therefore may differ from one event to the
next.
1. Select the check boxes of all applicable Violation Types. If the violation you are reporting is not presented in the list, there is
a choice of Other.
2. You should choose the “Other” violation type as a last resort and ONLY in cases where there is no appropriate violation from
the list that adequately describes the circumstances of the event. If you choose Other, you must fill in the details in the dialog
box provided or METS will not enable you to continue.
3. Select the Primary Violation Type by clicking on the corresponding radio button. You must select the Primary Violation
Type, even if you have chosen only one violation. When two or more allegations relate to an event, the Primary Violation
Type is generally the most serious allegation.
4. Choose the Product or Service by clicking on it if it is applicable. If you have selected a Product you must also select the
Primary Product, even if you have chosen only one.
5. You should choose Other from the Product or Service list as a last resort. When there is no related product or service,
choose not applicable. Use the Details text boxes to add more information regarding the product(s) as necessary.
6. Click the Save button when you are done updating. If you have not included information for any mandatory fields, METS will
produce an error message indicating the missing information. Scroll down and add the required information.

4.6 Related Events
METS requires a separate event to be entered for each subject involved in an event, however, events should only be related when
two subjects are connected to a single event trigger, such as a customer complaint or civil claim, and not when the member receives
more than one regarding the same subject.
For example, a customer may complain about his account performance over several years and name his original approved person,
his current approved person, and their branch manager. This should be three, related, events. However, if you receive two
complaints, from two different customers, regarding the same approved person, you should not relate the events.
To relate an event:
1. Click the Edit button to enable the related events section of the Event Report Form;
2. Select the appropriate METS Reference Number, which appears at the top right of each Event Report Form, from the dropdown
menu, click Add.
3. Repeat as necessary for additional events.
4. If you relate an event in error, you may delete the relationship between the two events by checking the box to the left of each
METS reference number and clicking the Delete button. This deletes only the relationship, not the event.
5. Click Close when you are done.

4.7 Status of the Event
1. Initiation Date: Set at the time the event was created. You may correct it if you made a mistake at the time of event creation.
2. Status
• Pending: In the process of resolution.
• Concluded: When the event has been resolved, you should update the status to concluded
Most Event types are created with the Event Status set to Pending. There are 2 exceptions: Denial, Cancellation, Suspension or
Termination of Registration or Approval; and Termination, Compensation Paid to Client and Internal Discipline (not otherwise
reported above). These are both automatically set to Concluded because these events have already come to a conclusion by
triggering of the reporting obligation
3. Status Detail: Additional details regarding either the ongoing status or resolution of the event that you are required to add to
most event types.
Note: If the Status Detail you are choosing is not presented in the drop down list, please click Other from the list and fill in the free-
form text box that becomes available.
4. Date of Resolution: This field is required for all concluded events. This relates to the primary event not any actions stemming
from that event.
For example, if a customer complaint is reviewed and found to have merit, the member may make an offer of settlement. The date
this offer is made is the date of resolution. (Acceptance of the offer is not required).
Even if the member also terminates the approved person in relation to this complaint two weeks later the date of resolution does not
change.

4.8 Member Response
• Member Reference is available for the Member’s use and completely at the Member’s discretion.
• Contact person for report: Add the name, phone number, and email address of the appropriate Member contact for this
Event in order to assist an MFDA staff member who requires more information relating to this event.
• If there is an Internal Investigation, enter the Start date of the Investigation. Once the investigation is concluded, enter the end
date as well.
• Enter all Member Identified Allegations, including the primary allegation. While similar allegations are listed in the Nature
and scope section, this area allows the member to show that more, less or different allegations may be identified by member
staff than may have been alleged by a customer.
For example, the member may receive a complaint alleging theft but, upon further review, the member finds that a fund merger
caused the customer to misread a statement and believed her money had been removed from her account.
As a further example, a customer may raise suitability concerns while the member may find that the approved person had pre-
signed forms in the client file and note the falsification allegation in this section.
• Report the Finding of the Investigation by choosing it from the drop down menu.
• Choose the Disciplinary Action, if any, that was taken as a result of the event from the drop down menu.
4.9 Comments
If you have any Comments, information or updates to add (that don’t fit or aren’t appropriate in other fields), you may type or copy
and paste them into the Comment space.
Adding extra information to your METS report is always useful for any MFDA staff reviewer.
The comment field can contain approximately two standard pages of text.

4.10 Document Audit
The Document Audit section of the Event Report Form displays the history of changes that have been made to an event. This
includes the Revision Number, the date and time the event was edited, what Event Report Form section was edited and the user who
performed the editing.
Note: The revision number of an Event starts at 0. Any time an Event is locked and reopened, the revision number of the event goes
up one, from 0 to 1 and so on. Each time an Event is reopened a copy of it, as it was when it was last locked, is stored in the Audit
history. This previous version will appear as a link which, when clicked, will open the previous version of the event Report.
5 LOCK AN EVENT
Locking an event is important as it demonstrates to MFDA reviewers that the Member has completed its work relating to the event
and does not expect to make further updates.
The normal process of completing an event report is to change the status from Pending to Concluded and then lock the event.
The Lock button is only visible when the event status is Concluded and appears at the bottom of the form below the document audit
section.
When the Lock button is clicked, the system will check that all mandatory fields are complete.
If any mandatory fields are incomplete an error message will appear to remind you to fill in missing fields.

6 REOPEN AN EVENT
If you receive additional information relevant to an event after locking the event you may re-open it.
The Reopen button is visible only when an event has been locked.
Once reopened, an event should be Locked again after any updates have been completed.
The system will retain a copy of all data in the event report form as it appeared when initially locked.
To view the previous version, open the Document Audit section of the Event Report Form.
An event that has been reopened will display a revision number in the Event Report Form header:
7 CANCEL AN EVENT
If you have entered a duplicate event you must contact MFDA METS Support.
Only the MFDA Administrator can cancel or delete an event.
A cancelled event is virtually deleted for Member users and for most reports. The cancellation is noted in the document audit.

8 SEARCH AN EVENT
Search and Report screen tips:
The basic search fields are Complainant name, METS Reference Number, Event Type, Violation Type, Date of Initiation, Date
Recorded, Status, and Subject Type.
You can use the basic search fields or you can click the Advanced Search checkbox to open up more search options.
Any time you click a Selection radio button; a list of items for that category will be displayed. Check the boxes of the items that you
want to include.
If you choose an option that requires more information, you will be provided with additional search fields for that selection. For
example, if you choose to search for Events with Subject type: Individual, additional dialog boxes will appear to permit you to be
more specific in your search for an Individual

Events that meet all search criteria will be listed in the search results.
Click the METS Reference Number of a particular event to view its Event Report Form.
If you want to go back to the Search An Event screen to change your search click the button.
9 VIEW EVENT DETAILS
Every METS user has the ability to view Event Details. Users can only view Event details of events that pertain to their Member.
To View Event Details:
1. Search an Event.
2. In the search results, click on the METS Reference Number to open the Event Report Form for that event.
3. If an event is not Locked, users may be able to edit event reports based on their role.

10 REPORTS
The Reports link on the left side of the METS screen allows you to create Reports by Subject, Status, or Type.
10.1 Event Report by Subject
Choosing Event Report by Subject enables you to view a report highlighting events organized by Subject Approved persons or
branches.
10.2 Event Report by Status
Choosing Event Report by Status enables you to view a report organized by Pending and/or Concluded events.
10.3 Event Report by Type
Choosing Event Report by Type enables you to view a report organized by Event Type.
10.4 Run a Report
1. Click the Reports link on the left side of the METS window.
2. Choose a Report type: Subject, Status, or Type.
3. In the “Print Report for” section, customize your report using the available fields. (Fields vary slightly according to the Report
type you have selected.)
Click the Selection radio button if you want to narrow the scope of your report. The selection list will open. Hold down the
Ctrl key and click to select multiple items from the list.
4. Select either the Print Format or File Format radio button.
If you choose Print Format, a separate window called the Report Viewer will open. From there you can choose to export
the results to different file formats. Refer to the Report Viewer section for more information.
If you choose the File Format option the report will be formatted and exported to excel automatically. The page headings will
be inserted as columns in order to allow you to work with each row independently. Additionally, the column headings will be
in the first row of the spreadsheet. This option is the most effective if you are using the report information for analysis or as
input into another report or document.

10.5 The Report Viewer
If you run a Report using the Print Format option a separate window, called the Report Viewer, will be open to display the Report.
Click the Navigation Buttons to view the entire report. You can also type a specific page number into the dialog box.
Change the display size of the Report, which by default is set to 100%, by selecting a value from the drop-down list.
The report displays the parameters you set when you ran the report to avoid confusion when reviewing the report at a later date.

10.5.1 Export a Report to a File Format
You can save or print a report as it appears in the Report Viewer by exporting it to a file.
1. Select a file Format from the drop-down list in the navigation bar at the top of the report.
For example, Acrobat (PDF) or Excel formats
2. Click Export.
A window will appear prompting you to Open or Save the file.
3. Choose Open. (You can always save it later from within the application).
The file will open into the default application for the file format you have chosen.
4. You may use application functions to customize the report, if required.
5. Finally, from the application, you can select Print to print the report or Save to save the file in the applicable format.

11 Member Administrator Section
11.1 USER SECURITY ROLES
Within METS, levels of access to information and the functional capabilities of the system are defined by the "role" assigned to each
user. Member personnel have access only to files associated with their respective Member(s). Refer to the table below for the list of
functions available to each role.
The Reader can search, view, and run reports on events pertaining to their Member. They cannot create or modify events.
The Author can do everything that a reviewer can but can also create, edit, lock and re-open events.
The Administrator can do everything that a reviewer can as well as create and update users and is the main contact person
between the Member and the MFDA for METS related issues.

11.2 Manage users
Only Administrators can manage user accounts for their respective Members. This is done through the “Users” function of the
Members’ Only Website.
You link to the Users function separately at https://www.mfda.ca/manageusers. You Login to the Manage Users function as you
would to the METS event creation site.
11.3 Add a New User
• To add a new METS user, first click on the USERS link in the main navigation bar. Click the New User tab.
• Complete all of the fields and set a temporary password for the new user. Ensure that “METS User” is selected for the User
Role drop down.

• To add a role to the New User, select a role and Member. Ordinarily only one Member will appear, however, if the MFDA
administrator has related more than one Member, you will have to ensure the correct Member and role is selected for each
user. Click Add Assignment to apply the role to the user.
• It is possible to assign to the user different roles at each Member, for example, a user may need to create events for Member
A but only view events at the related Member B for review or supervision purposes. In such a case, the user should be given
the author role at Member A and the reviewer role at Member B.
• Once all roles are assigned, click Save.
NOTE: this new user must be activated before they will be able to login to METS. (How to activate user accounts.)
11.4 Edit an Existing User
1 From the USERS section of the Members’ Only Website, click on the Search tab.
2 To view all user accounts, simply click search and the first 15 users will be displayed, if your Member has more than 15 users,
use the search criteria to narrow the results to the individual you wish to edit.

3 To activate or inactivate a user select or deselect the Active checkbox for the user from the grid.
4 To share administration of a user with Members’ Only Website Administrators at your firm, enable (check) the Is Profile Shared?
Checkbox. Doing so will permit Members’ Only Website Administrators at your firm to change most User Profile fields and add /
remove Members’ Only Website user roles. However, only METS Administrators are permitted to add / remove METS user
roles.
5 Click the notepad to edit this user account. Click the eye to view the details without editing.
6 Make changes to the fields as required.
7 Click Save and to save your changes.
8 If you need to add or change a role, you can do so now.
11.5 Activate / Inactivate a User account
1 Click on Search tab of the USERS section.
2 To view all accounts, simply click search and the first 15 users will be displayed, if your Member has more than 15 users, use the
search criteria to narrow the results to the individual you wish to edit.
3 Click the Active checkbox on or off to activate / inactivate the account.
4 To activate a user, put a check the Active box. If you wish to inactivate a user, ensure the box is empty.

12 Special - Member Selection Screen
Most METS Users will have access to METS for one Member only, however, when two or more Members are related at a corporate
level, the Members may request that certain users be permitted to have access to METS for more than one related firm.
If you have access to more than one Member, after successful login, you are presented with the list of the related Members available
to your login ID. You must select a Member in order to continue in METS.
If you do not select a Member and click on another menu button (except Logout), a pop-up message will appear reminding you to
select a Member.
The Member Selection Screen will also appear if you click the Change Accounts link on the left side of the METS screen.
If the Change Accounts link is greyed out it means that you only have access to one Member.

13 TROUBLESHOOT -Disable Pop-Up Blockers
The most common problem that METS users encounter is the interference of pop-up blockers. There are several ways to deal with
this issue, and you may use any method, but we recommend and support only one:
Adding METS to your Trusted Sites List
1. Open Internet Explorer.
2. Under the Tools menu, click “Internet Options…” and then click on the Security Tab:
3. Click on Trusted sites, and then on the Sites button.
4. In the Add this web site to the zone dialog box, type https://www.mfda.ca/mets and click Add.
5. Click OK.
Note: You should consult your member’s Information Technology staff for their guidance in this area prior to making any
changes.
