Our modern societies are suffering the increase of elderly population while at the same time
social security and health costs must be cut down. In order to avoid the need of special care
centres, both to reduce costs and to improve elderly people quality of life, the actual trend in
Europe and North America is to encourage elderly to stay living autonomously in their own
homes as long as possible. However, due to health problems mainly, elderly are more subject
to problems and need of assistance in their daily life. For most of them, living alone is not
acceptable, and generates insecurity, fear, restriction in their activities and anxiety or concern
from their carers or families. Professional carers may of course help elderly in their daily life,
but they never can give a 24 hours service a day. Another mean to support elderly is to
provide them adapted and reliable telemedicine systems, to make them more confident, to
support them in case of emergency, in fact to improve their quality of life. The product
presented in this paper contributes to this objective, since it provides automatic fall detection
and activity monitoring both for indoors and outdoors activities, associated to a complete call
centre for medical monitoring of the patient as well as to provide support and manage
emergency situations.

The fall detector and activity monitoring device is being developed within the European
CRAFT project HEBE, with centres and SMEs from Spain, France and Greece. It is
constituted by a small mobile module worn by the user during its daily activities, and a call
centre to receive and manage information sent by the mobile module. Three main functions
are by now implemented in the product: automatic fall detection (with alarm sent in case of a
fall), daily activity monitoring (with daily report of user’s activity for medical monitoring),
and user localization for emergency coordination.

In order to allow an indoor and outdoor use of the system, mobile and wireless technologies
have been used. Indeed, the mobile module incorporates GSM/GPRS together with stack
TCP/IP to send data (activity monitoring and alarm) between the mobile module and the call
centre. For user localization, the system incorporates a GPS receiver.

The module works in an autonomous way, without intervention of the user. In normal mode, a
daily activity report (degree of activity during the day) is sent to the call centre for medical
monitoring. In case of alarm (due to a fall or abnormal activity), an alarm message is sent to
the call centre with all information required to deal with the emergency. Furthermore, the user
has the possibility to cancel or to set manually an alarm, and the call centre offers the
possibility to retrieve position or activity report at any time.
