Modified Program Application Form
Mechanical Engineering with Mechatronics & Embedded Systems Option
Instructions: Fill out both sides of this form. On the Proposed Academic Schedule (reverse side), label your current term with
“Current.” Mark with a check (√) those courses and work terms you have already completed. Strikeout those courses that you
plan to take in a different term than currently listed. Write in the courses and work terms you propose to take out of the usual
sequence. You must detail the complete sequence of your program, including all missing program requirements, work terms,
and out-of-sequence terms and courses. Failure to include all of this information may result in denial of your request.
After completing this form, submit it to your department advisor. Upon approval of this form by your department, it will be
forwarded to the BEng/BSEng Student Programs Committee for consideration. The committee meets on the last Thursday of
each month. You will receive written notification of the outcome.
You are responsible for satisfying course prerequisites and resolving timetable conflicts that result from following a
nonstandard schedule. Any subsequent changes in your program require submission of a new Modified Program Application
Form.
