What is KCHAIN?
Kane Community Health Access
Integrated Network (KCHAIN) was
awarded a federal Healthy Communities
Access grant (HCAP) to build an
integrated health provider network to serve
the growing need for affordable health
care to low-income residents.

KCHAIN Mission
The mission of KCHAIN is to increase
access to primary and specialty care,
improve the quality of health care, and
integrate key health system functions in
order to leverage existing resources,
including private sector specialty care, to
produce cost savings that will be re-
invested into the health safety net system.
KCHAIN Program Goals
1. Increase access to primary care for our
target population through encouraging the
establishment of a “medical home”.
2. Increase access to Specialty Care by
developing a centralized Specialty Care
Referral Network.
3. Increase pharmaceutical access for KCHAIN
clients through a centralized Pharmaceutical
Access Program.
4. Create Management Information System (MIS)
which allows KCHAIN partners to compile and
share data
Increasing Access to Primary Care
KCHAIN leverages resources of the Federally
Qualified Health Centers (PHS 330s) and other
community-based health care clinics to provide
sustainable, high-quality, culturally competent,
primary care.
A Federally Qualified Health Center (FQHC) is a
non-profit provider of health care in a medically
underserved area.
Created by Congress and generally receive grant
funds to provide care for the uninsured.
Different from private practice by management
structure and the broader, community based services
they provide.

Increasing Access to Specialty Care
Proposed Outcomes
Establishment of a centralized referral system that will
effectively fulfill 75% of the requests for specialty care
Establishment of a provider network that will include
70% of the specialty care providers in Kane County
Establish a process of ongoing recruitment and
retention of specialty care providers that allows the
physicians to care for patients in the setting that they
prefer
Work with the community via IPLAN to strategize for the
additional layer of specialty care delivery by means of
the fixed specialty care clinic sites throughout Kane
County.

Provide Kane County residents with information regarding
the discount pharmaceutical resources available.

completion of Prescription Assistance Program
Applications.
Develop a Pharmaceutical Assistance Fund to:
Bridge the 30-45 day gap that exists with most Pharmaceutical
Assistance Programs sponsored by Pharmaceutical Companies
Providing emergent medications such as antibiotics and pain
medications
KCHAIN Voucher Program
Vouchers are redeemed at Kane County
Jewel/Osco Pharmacies.
10 Pharmacies in Kane County
24 hour Pharmacy located in Aurora, St. Charles, and Elgin
Vouchers have a 20% Co-pay
Eligibility Requirements:
Kane County Resident
200% or below Federal Poverty Level
No insurance coverage-Private or Medicaid
Has a Primary Care Provider/Medical Home
Program Rules:
First time applying to KCHAIN for this medication in the past
year
Medication is less than or equal to $75.00
PAP application completed and submitted for maintenance
medications.
KCHAIN MIS
The KCHAIN Management Information System will allow
KCHAIN to compile data from all of the KCHAIN partners.
It is important to note that this technology will allow Kane
County to, for the first time, have a countywide effort to
integrate and centralize health care information.
The KCHAIN MIS will provide/manage the following health
services:
Provider referrals
Customizable health information tracking tools
Tracks client appointments and events
Assures HIPAA Compliance
Pharmaceutical Assistance Programs
Benefit Linkage (using Real Benefits)
Centralized database of client data
Easy-to-use reporting mechanism
Maintains patient history for audit purposes
