Social Security Number (SSN) The Social Security Number (SSN) aims at uniquely identifying a subscriber, nationally. SSN will be allotted to each member ensuring that a member does not have more than one SSN. This number will be permanent and will be used for pension purposes also. This will address the primary problem of a mobile workforce. Requirement of SSN for the employees working for an establishment under a Pilot Office Any service to the employees working for an establishment under a Pilot Office of EPFO will be provided only on the basis of the SSN of the member. Any return of a newly joined employee can be filed only on the basis of SSN. Any fresh PF Account number will not be accepted by the system. Role of Employer/Establishment As a first step, it is mandatory for every employee of the establishment to fill up the form and get an SSN allotted. All the service to the member will be provided by EPFO only through the SSN. This is a one-time requirement and all the information asked for has to be furnished accurately. Ask the employees to read the form and the instructions carefully before filling it. Alternatively, the HR department of the establishment can fill up the forms and send it to the EPFO Office. In case there is a requirement of more forms, please contact the nearest EPFO Office. Guidelines for filling the forms will be provided with the forms and is also given below. Distribute the forms to your Employees with elaborate guidelines on filling up the forms. Use the “SSN-An Employees’ Perspective” presentation, supplied by EPFO to create awareness among your employees. To ensure that all the employees of the establishment get an SSN, the establishment should take following steps: a. For any new employee if not already having an SSN, joining the establishment the SSN form duly filled in should be taken from him/her, and should be submitted to the EPFO Office. The allotted SSN will be intimated within a few days. The said SSN should be used in the return. b. Whenever any employee who had earlier left the service comes for any attestation of his claim, a duly filled SSN Form should be taken from him and the SSN Form as well as the claim form should be sent to the EPFO Office. c. For all the existing employees, it should be checked as to who have not filled the SSN Form and their SSN form should be collected and submitted for allotment of SSN immediately so that in future the SSN may be quoted in the returns and for all other purposes. Current PF Account Number. Please enter the PF account number or pension account number which ever is applicable. If you are not sure about your number, take the guidance of your employer or the authorized person in your establishment. 2. Full Name of Subscriber a. If you have a single word in your name, then please enter that word in the first name item only. b. If you have two words in your name, then please enter first word in the first item and second word in the last name item only. Leave the middle name item blank. Read the examples given in the form. c. If you have multiple words in your name, then enter first word in the first name item and last word in the last name item and the remaining words in the middle name item. d. Expand the initials fully. e. Do not use nicknames or short names. f. Do not use any title like Dr, Mr, Ms, Mrs, Shri, Smt, etc. or abbreviations. g. First name should not be left blank. h. If middle name is filled, then the last name must also be filled. 3. Father’s Full Name a. If Father’s Name has a single word, then please enter that word in the first name item only. b. If Father’s Name has two words, then please enter first word in the first item and second word in last name items only. Leave the middle name item blank. c. If Father’s Name has multiple words, then enter first word in the first name item and last word in the last name item and the remaining words in the middle name item. d. Expand the initials fully. e. If Mother’s Maiden Name has a single word, then please enter that word in the first name item only. b. If Mother’s Maiden Name has two words, then please enter first word in the first item and second word in last name item only. Leave the middle name item blank. c. If Mother’s Maiden Name has multiple words, then enter first word in the first name item and last word in the last name item and the remaining words in the middle name portion. d. Expand the initials fully. e. Do not use nicknames or short names. f. Do not use any title like Late, Dr., Ms., Mrs. Smt, etc) or abbreviations. g. First name cannot be left blank. h. If middle name is filled, then the last name must also be filled. g. Only the mother's maiden name should be entered. 5. Sex a. Please darken the applicable circle fully. 6. Date of Birth a. Enter the date in the DD-MM-YYYY format. b. You can contact the ECR for guidance, if you are not sure of the exact date of birth. c. The ECR should check the date of birth with the date of birth in the EPFO records. If the two do not match, then, the ECR should follow the instructions provided by EPFO Head Office in this regard. d. The date should be filled in DD/MM/YYYY format only. e. Ex - 01/09/1962 is an acceptable date. f. If the month or day is a single digit (1 to 9) then the day or month should start with 0 (Ex- 01 and NOT 1, 07 and NOT 7). Similarly the year 62 should be written as 1962. 7. Place Of Birth a. Place of birth cannot contain numerals. Example “24 Paraganas” must be entered as “TWENTY FOUR PARAGANAS”. 8. Nationality Of Applicant. a. Please darken the applicable circle fully. b. If the Nationality is not Indian, full country name must be given in the space provided. Abbreviations like UK, USA, RSA etc are NOT allowed. c. In case of displaced persons from Pakistan or Bangladesh, the country name will be INDIA. In case of displaced persons from Srilanka, the country name will be Srilanka only. 9. Have you ever been known by any other name? a. Write your erstwhile name if any, in this item. (No nicknames or short names) b. Married women must enter their maiden/given name here. c. Instructions given under item no. 2 for writing your name are applicable here also. 10. Correspondence Address a. Please read the caption given against each line of boxes before filling and then fill accordingly. b. Superscript and subscript like 1st, 2nd or IInd (for Floors and Phases) should not be used. 2nd should either be written as “Second” or “2nd” the “2”, “n” and “d” appearing in individual boxes. If Roman characters are used, only one character must appear in one box. Thus “II” will be written in two boxes with one “I” in each box. Commas, hyphens, slashes etc must also be entered in separate boxes and NOT along with other alphabets. c. In case any item is not relevant then leave it blank. 11. Permanent Address a. If your permanent address is the same as correspondence address then, you should darken fully the circle provided and leave the individual items under this head blank. Otherwise The same rules as in column 10 (Correspondence Address) applies here also for filling the items. 12. E-mail, if any a. Please enter the e-mail (if you have one) in block capital letters. 13. Name as it would appear on card a. In this item you can use initials/short names Do not write nick names here. 14. Father’s Name as it would appear on the card a. This name can contain initials Do not enter nicknames here. 15. Declaration Signature/thumb impression a. The signature or thumb impression must not touch or cross the boundaries of the box provided. b. Sign or place your thumb impression horizontally in the center of the box. FAQs on SSN Question 1: Is it compulsory for the claimant/subscribers to furnish SSN in the claim form for claiming benefits from the EPFO? Answer: Yes. Question 2: In case the member does not have the SSN, then how he will file the claim forms? Answer: The member will be required to submit SSN forms duly filled for allotment of SSN along with claim form. Question 3: If SSN was allotted to the member but he/she does not remember his/her SSN. What should he/she do? Answer: Such P.F. members will be required to produce the identity proof to the satisfaction of PRO to help the PRO in locating the SSN. Question 4: Who will be responsible for submission of SSN form to the EPFO- employer or employee? Answer: The employer as well as employee will be responsible for filling of the SSN form and its submission to EPFO. Question 5: What will be process of updating SSN related information? Answer: The member will have the facility to submit updated information in the SSN form through his employer for incorporating the changes in the database. However, he will not have any option to change the static key fields. Question 6: Whether SSN will also be issued to the dependant family members? Answer: No, the SSN will not be issued to the dependant family of PF subscribers. Question 7: What will be the fate of SSN after final settlement of claim of the P.F. member? Answer: After final settlement of claim of the members SSN will remain in the system. In case the member re-joins another establishment covered under the Act, the SSN will be linked to the BN of that establishment. Question 8: In case of death of P.F. member, whether SSN of such members will be transferred to the dependant’s family member? Answer: No. However the family members will be required to furnish SSN of the member in the claim form for claiming benefits from the EPFO. Question 9: P.F. member has furnished SSN in his claim form. Is it compulsory for him to get his claim form attested? Answer: Yes. The P.F. member is required to get the claim form attested from the authorised official specified for this purpose for settlement of claim. Question 10: How the SSN intimated to the P.F. members? Answer: The SSN will be intimated to the P.F. members through his employer. Question 11: Will there be a facility for taking photograph of the member in the field offices for allotment of SSN? Answer: Yes. There will be a facility for taking photograph of the member in the field offices of the EPFO. In such case the member will have to visit the EPFO Office. Question 12: Who will responsible in the field offices for answering SSN related queries of the P.F. members? Answer: PRO in the field offices will remain available for answering SSN related queries of the P.F. member. He will be able to clarify all the doubts once the member establishes his identity up to the satisfaction of P.F. Question 13: How the SSN will be allocated to the existing pensioners both member pension cases as well death cases? Answer: Based on the database of the pensioners available in the EPFO, the SSN of the existing pensioners shall be generated and communicated to the pensioners. In case of the family pensioners SSN of the deceased member will be communicated to the beneficiary. Question 14: If the employer refuses to attest the SSN form of the member then what is the immediate remedy available to the P.F. member? Answer: In such cases the member will be required to submit the SSN form duly filled to the EPFO Office for further action.
