									        HOW TO APPLY FOR SMOKE ALARM SUBSIDY VOUCHER

STEP ONE: FIND OUT ABOUT THE SCHEME AND IF YOU ARE ELIGIBLE
 Applicant receives information about Queensland Fire and Rescue Service
 (QFRS) Smoke Alarm Subsidy Scheme. Check you meet the criteria.

STEP TWO: SELECT A SYSTEM AND SEEK A QUOTE FROM SUPPLIER
       Choose a smoke alarm from one of the systems outlined in the
        information sheets provided by QFRS. This must have a strobe and/or
        acoustic (sound) warning and vibrating pad.
       Contact one of the approved suppliers of the system you have chosen
        and ask them to provide you with a written quote with reference number.
STEP THREE: PREPARE YOUR SUBSIDY APPLICATION TO QFRS
 Check you have:
    signed confirmation from a General Practitioner, doctor or hearing
      practitioner that your level of hearing loss may prevent you from hearing
      a standard smoke alarm, or a previous assessment no more than three
      years old that you are profoundly deaf;
    a copy of your Centrelink Pension Concession or Health Care Card,
      Department of Veteran’s Affairs White or Gold Repatriation Card;
    an original copy of your quote (keep another copy for your records).

STEP FOUR: SUBMIT YOUR APPLICATION MATERIAL
       Mail information (including your contact details) to Queensland Fire and
        Rescue Service at: ‘Smoke Alarms for Hearing Impaired Person’s
        Assistance Program, GPO Box 1425 Brisbane Queensland 4001’.
       There is NO form required.
STEP FIVE: APPLICATION PROCESSED
       QFRS will assess your application.
       You will be contacted if you do not meet the eligibility criteria or if you
        have not provided all of the required documentation.

STEP SIX: VOUCHER POSTED TO PRESENT TO SUPPLIER.
       QFRS will mail a numbered voucher to you.
       You will need to take this to the supplier with a copy of your original
        quote as well as either your driver’s licence, concession card, passport
        or birth certificate for identification purposes.
       The supplier will check your voucher details against their QFRS approval
        letter. If it matches, they will accept it as payment of the amount stated
        on the voucher. You will have to pay for any amount above the subsidy.
STEP SEVEN: INSTALLATION
       You must also make your own arrangements for having the system
        installed. You can try the Home Assist Secure Service on (07) 3227 6816
        or 1800 642 902 or email communityhousing@housing.qld.gov.au to see
        if you are eligible for assistance. Alternatively, call a certified electrician
        listed in your local telephone directory.
