Summary
On 5-2-05 a staff member came to the security office with a student who was injured. The
student advised that he was moving a bookshelf in L-182. A computer monitor, that was on the
top shelf, fell and struck him in the head. He sustained several small cuts to his scalp. The
student was treated by a faculty member of the HCC Nursing Department and refused further
treatment. He was transported home by his mother. The staff member was a witness to the
incident. An Employee's Incident Report Form was completed by another staff member.

Summary
Security was dispatched to the Athletic & Fitness Center in reference to an injured subject..
Investigation revealed that the individual was working out in the weight room. As he backed up,
he tripped over an incline bench and fell to the floor. He fell on his right hip. He sustained
unknown injuries, as he was unable to move his right leg. An ambulance was called and
transported the individual to Howard County General Hospital.

Summary
Security was called in reference to an employee who was feeling ill. Security was advised that
she needed an ambulance so 911 was called and an ambulance was dispatched to the scene.
Upon arrival at the scene, Security found the ill employee in room 212 of the Library. Her
complaint was that she felt hot and tingled in her face which quickly spread down her whole
body. She advised that it felt like a bad sunburn. Subsequently, she broke out in a rash which
turned her skin red. After Security's arrival, the employee stated she was feeling better. The
ambulance arrived and transported her to the hospital.

Summary
Security was responding to a call to report to Lot A. Upon arriving, a student claimed that she
parked in a parking spot and upon trying to exit, the piece of rubarb (spkie) caught her muffler
causing damage to the bumper. While arriving, Security observed a tow truck driver trying to
pull the car out by placing a tall piece of wood under the left rear tire so the car could go over
the piece of wood. This also caused the damage to extend more. The student and the tow
truck driver took pictures of the damage with camara phones. The student then drover her
car to the car dealer.

Summary
A faculty member came in the double doors from the Quad and when she stepped into the
hallway, she slipped on the floor. She hurt her left hip and left heel of her hand. She did not
feel that she needed to go to the hospital. There was no rug mat to wipe her feet on.
Security called Plant Operations to ask them to bring one as soon as possible.

Summary
An individual tripped while walking up the staircase from the lobby of the ILB to the 3rd floor.
She sustained minor injury to both knees and her left elbow. She refused treatment. There
were no hazards present and it appears that she tripped through inattention.
