Copies of contractor qualifications and passports are held in filing cabinets Loss of key systems, e.g. through IT failure/software supplier failure (not Exeter System) e.g. Unable to access internet to make pharmacy/ophthalmic payments

Request Landlords of the building to assess ways in which security of the building can be improved. A person has been charged with two previous break-ins. Consider purchasing ‘locking cabinets’ and ‘smart water’

Avon IM&T Consortium 12 responsible for business continuity planning in place for KSH. Would need to access internet via another source e.g. Another PCT base. Agreement with South Gloucestershire PCT to access internet from their base.

Failure of mailing machine

PCT’s do not wish APCSA to work collaboratively across health community

Insufficient System Documentation about internally developed procedures. This is not a problem while staff that developed the procedures are still in post, but is a problem if those staff leave the Agency Staff shortages – and inability to Multi-skilling & prioritisation 12 employ commercial agency staff Agency will have to assess and no availability of Bank staff services that cannot be provided and advise PCTs Excess demand Monitor demand and identify 9 risk areas may result in reduced ability to satisfy customer demand – unmet targets

Disaster recovery suite available at NHS Connecting for Health (NHSCFH). Avon IM&T have provided a solution for access to the Exeter System and has been successfully tested. Now working with the NHSCFH to secure BACS process can be run without KSH IT Network. Ensure maintenance agreement 9 in place with PFE, maintenance agreement gives 1 day call out. Use alternative machine in Patient Data/Child Health PCT responsibility additional 9 costs may be incurred as a result. PCTs should discuss at Board meetings. Many procedure notes already 9 documented. Ensure they are regularly reviewed.

Finance
Knowledge held by an individual is lost should the individual leave. Cover is also needed for unexpected sickness Procedure notes to be written as soon as processes are simplified. Ensure procedure notes are tested and another member of staff trained in their use. Contracts
A PCT is unable to provide Clinical Governance Managers authorisation of now administer process within references/decision making due PCTs to clinical governance lead being unavailable.
