					Oklahoma Campus Compact VISTA PROGRAM
Monthly Project Site Report
PURPOSE The purpose of the Project Site Report is to:  Document progress toward achieving project goals and objectives;  Provide a self-assessment tool to projects to promote continuous improvement;  Identify technical assistance needs; and  Collect information for distribution to the public COMPLETION AND SUBMISSION GUIDELINES  A report is due on the 5th of every month.  All reports are to be send by email to Risa Johnson – Rjohnson@osrhe.edu.  No faxes will be accepted.  If a section of the report is missing, an explanation is required. Recruitment Please describe efforts made toward recruitment of AmeriCorps*VISTA members. Include any best practices developed, or specific challenges faced. Please describe steps you are taking to ensure a smooth transition between outgoing and incoming AmeriCorps*VISTA members if applicable.

Resource Development Describe the resources recorded in the Data updates section for this reporting period. The description should include the source and the use of each cash or non-cash donation. In addition, the description should include the activities of community volunteers and the strategies used to leverage community volunteers. In addition, please identify the AmeriCorps*VISTA member(s) responsible for obtaining the resource.

Sustainability Describe specific organizational strategies that are in place to sustain the results of the project. What is your plan for transitioning your project away from AmeriCorps*VISTA resources?

Other Accomplishments Describe in detail any additional project or member accomplishments that are not reported within the work plan. Include any awards received.

Stories Include stories that would best communicate to the public how AmeriCorps*VISTA members get things done in your community. Particularly helpful are stories that include numerical results and sustainable solutions.

Attachments and Links List the attachments, or links to documents, that will be submitted as part of the Progress Report. Examples of attachments include copies of press clippings, flyers, letters or other documents that relate to the members’ activities and accomplishments.

In addition to the standard data requested by AmeriCorps*VISTA from state projects for the quarterly progress report, performance data must be given for compliance with the designated state project goals. Campus Resources Please use the following information to complete the attached State Goal Performance Measures     Use the attached forms, of which there are six – one for each Performance Measure The Performance Measure is a number, or percentage (%) as appropriate, to be entered in the blank labeled ACTUAL under Indicator, in the third box on each page. It is not required to enter data for every Performance Measure every month, but at least three or four Performance Measures should be given each month. Please enter 0 for any Performance Measure not undertaken, and submit a total of six forms.

Activity: Develop a survey to be used as an assessment tool for measuring the needs of community agencies, conduct on-site visits
or phone contacts with the appropriate community agencies, and orient them toward collaboration with service-learning sites.

MEMBER: Whenever possible, leave requests should be completed at least a week in advance of your first day of leave. For sick leave, complete this form following your return from any sick days. Keep a copy for your records and send originals with your timesheet on the appropriate date. Attach a copy to the monthly report.
