I have audited the Electoral Campaign Return of (candidate), a candidate in the election
of a member to serve in the House of Commons of Canada held on the 27th day of
November, 2000 in the Electoral District of ……………, and reported thereon under date
of (date of the auditor’s report).

Pursuant to subsection 453(2) of the Canada Elections Act, I am required to complete a
checklist in the prescribed form. The information set out in this checklist was obtained
during the aforementioned audit. No procedures have been carried out in addition to those
necessary to form an opinion on the Electoral Campaign Return.

This report is to be used solely to satisfy the requirements of the Canada Elections Act
and should not be referred to or used for any other purpose.
