BUREAU OF CHRONIC DISEASE PREVENTION & HEALTH PROMOTION Secretary’s Quarterly Performance Measure Review Contract Year 2003– 2004, Second Quarter/January-March PHYSICAL ACTIVITY ASSESSEMENT & COUNSELING SERVICES PROVIDED BY COUNTY HEALTH DEPARTMENTS Explanation of Measure: Physical activity assessment and counseling is a service provided by county health departments (CHDs) throughout the state. This service consists of an assessment of client’s current physical activity level and counseling on how to increase or maintain that level. This service is provided to create an awareness of the need for physical activity and to increase actual physical activity levels. The lack of physical activity is a primary risk factor for chronic diseases and conditions such as cardiovascular disease, diabetes, obesity, and osteoarthritis. 19,500 19,000 18,500 18,000 17,500 17,000 16,500 16,000 15,500 15,000 14,500 14,000 13,500 13,000 12,500 12,000 Oct-Dec 2003 Jan-Mar 2004 Apr-Jun 2004 July-Sept 2004 Physical Activity Assessment & Counseling Services Quarterly Goal Trend:. A total of 18,792 physical activity assessment and counseling services were reported this quarter. This exceeds the quarterly goal of 16,000 and is an increase from last quarter. According to the data for this quarter, 46 of 67 CHDs are coding this service to at least one of the applicable program components. This is an increase of 4 CHDs reporting this service over the 42 CHDs that reported providing this service last quarter. The total number of services reported by a CHD this quarter ranges from 1 to 3,025. Data Source and Limitations: The Health Management Component Reporting System (HMC) is the data source for this measure. Physical activity assessment and counseling services coded in the following program components are counted for this measure: Sexually Transmitted Disease Services, Acquired Immune Deficiency Syndrome Services, Women, Infants, & Children Services, Family Planning, Improved Pregnancy Outcome, Comprehensive Child Health, School Health, and Comprehensive Adult Health. A limitation of this data source is that delivery of this service may not be reported in HMC or this service may be provided as a part of another service that is reported; e.g. nursing assessment/counseling or physical examination. Therefore all physical activity assessment and counseling services provided by a CHD may not be counted in this measure. Resources: CHDs do not receive funding specifically designated to provide physical activity assessment/counseling services. This service is typically provided in the course of providing another service such as nursing assessment/counseling or a physical exam. This service consists of individual and group education regarding prevention or management of chronic disease. Health education is provided to increase awareness and knowledge regarding chronic disease risk factors and mobilize support for making policy and environmental changes that promote a healthy lifestyle. 70,000 60,000 50,000 40,000 30,000 20,000 10,000 0 OctJanApr-Jun JulyHealth Education Class Participants Quarterly Goal Trend: There were a total of 53,837 health Dec Mar 2004 Sept education participants this quarter, which exceeds 2003 2004 2004 the quarterly goal of 40,000. The number of health education class participants this quarter is lower than last quarter. This is attributed to lower attendance at community presentations and fewer CHDs reporting services this quarter. According to the data for this quarter, 54 of 67 CHDs coded at least one of the services included in this measure. Data Source and Limitations: The Health Management Component Reporting System (HMC) is the data source for this measure. The number of participants is derived from a first time this year (FTTY) count for the following services: Community Presentation (7500), Nutrition Education Group (8010), Health Education Class (8021), Education Class (8020), reported in the chronic disease prevention and adult health program components; Cardiovascular Disease Education (8005), Diabetes Education (8007), and Cancer Education (8093) reported in the chronic disease prevention program component, and Smoking Cessation Counseling/Education (8027) reported in any program component. A limitation of this data source is that service delivery may not be reported in the HMC. Resources: The only funding CHDs receive from the Bureau of Chronic Disease Prevention and Health Promotion to deliver the services counted in this measure is specifically designated to implement Chronic Disease Health Promotion and Education (CDHPE) programs. These programs focus on policy and environmental changes to improve health outcomes but also deliver health education. The health education services delivered through the CDHPE programs represent only a portion of the total number of health education services counted in this measure.
