SUMMARY OF PROFESSIONAL QUALIFICATIONS FOR JOHN R. KINGSLEY, M.D.,F.A.C.S.,RVT VASCULAR SURGEON Board Certified by the American Board of Surgery The following is a summary of the professional qualifications of John R. Kingsley, M.D. A complete curriculum vitae is available from the office of the Alabama Vascular & Vein Center. PERSONAL: Dr. Kingsley grew up in Pensacola, Florida. He was educated in Florida and in the Navy, and practiced medicine in the Pacific Northwest for 14 years. He moved to Alexander City, Alabama in 1992, and later accepted a teaching position with Carraway Medical Center in Birmingham. More recently he returned to private practice and opened the Alabama Vascular and Vein Center. Dr. Kingsley and his wife have three children, a son and daughter at home in Vestavia Hills and a daughter in Portland, Oregon. He enjoys playing tennis (4.0 player), skiing in Colorado, relaxing on Lake Martin, and writing . EDUCATION & TRAINING Surgery training requires an undergraduate degree in College or equivalent pre-medical preparation, four years of medical school, and at least 5 years of surgery training. Dr. Kingsley completed his education as follows: Bachelor of Science Degree, University of Florida, Gainesville, Florida Doctor of Medicine Degree, University of Florida, Gainesville, Florida Intern in Surgery, Bethesda Naval Hospital, Bethesda, Maryland Resident in Surgery, Bethesda Naval Hospital, Bethesda, Maryland BOARD CERTIFICATION, AMERICAN BOARD OF SURGERY To achieve Board Certification in Surgery, physicians must have been in practice for at least two years, qualify for the examination by virtue of training and experience and professional activities, and successfully complete a written examination followed one year later by an oral examination. Beyond the initial certification, surgeons may take re- certification examinations every ten years. In addition, some surgeons may qualify for specialty certifications, again requiring the full examination process specific to that specialty. Dr. Kingsley is Board Certified and Re-Certified in both general surgery and vascular surgery. 1999 1996 1991 1988 1976 Re-Certified in Vascular Surgery Re-Certified in General Surgery Certified in Vascular Surgery Re-Certified in General Surgery Certified in General Surgery In the United States and Canada, there are only approximately 2500 Surgeons who have achieved Board Certification in Vascular Surgery, and only 800 who have been Re-Certified. In addition, Dr. Kingsley completed the requirements and examinations to become a Registered Vascular Technologist through the American Registry of Diagnostic Medical Sonographers. He is one of only a few U.S. physicians who are credentialed as an RVT, confirming his credentials and the importance he places on vascular ultrasonography. MEDICAL LICENSE In order to treat patients, a physician must hold a license granted by the state in which he practices. This requires successful completion of training, and/or Board Certification, and an examination. Alabama – licensed in 1976 (while in the Navy), re-activated in 1992. Oregon - licensed in 1978, now maintained as “out of state”. PRACTICE AND HOSPITAL APPOINTMENTS Dr. Kingsley practiced in the Navy, followed by his practice in the Pacific Northwest, and now continues his practice in Alabama. His current position is that of founder and director of the Alabama Vascular & Vein Center. He is on the medical staffs of several Birmingham area hospitals, but currently is affiliated primarily with Brookwood Medical Center where he admits his patients and performs his hospital based operations. He can provide consultations and surgery at the following facilities. Brookwood Medical Center Shelby Baptist Medical Center HealthSouth Medical Center Carraway Methodist Medical Center Medical Center East The Alabama Vascular & Vein Center is located in the Vestavia Hills City Center new mall complex. The newly constructed Vein Center opened in January, 2004. This Center includes vascular diagnostic equipment, a conference/education room, and two fully equipped procedure rooms which allow the safe performance of all vein treatments. AWARDS AND HONORS Over the years, Dr. Kingsley has been honored by his peers and by several professional societies. He has received numerous awards, a partial list of which follows: Recognized as one of the “Top Surgeons in America”, as recently published by the Consumers Research Council of America based in Washington, D.C. Previously elected Chairman of the Departments of Surgery for both the Norwood Clinic and the Carraway Medical Center. Elected to Executive Board of Alabama Vascular Society. Elected to Executive Board of Pacific Northwest Vascular Society. Selected President of the American Society of Outpatient Surgeons. Elected a Fellow in the American College of Surgeons. Elected a Fellow in the American Association for Vascular Surgery. Elected to the Southern Association of Vascular Surgeons. Appointed to the Editorial Board of newsletter Laparoscopic Surgery. Appointed to the Advisory Board of the Governor’s Committee on Ambulatory Surgery of the American College of Surgeons. Appointed to the Board of Directors of the Accreditation Association for Ambulatory Health Care. Selected Delegation Leader of U.S. Surgeons’ visit to Hospitals throughout China. Received the Navy Commendation Award. PROFESSIONAL SOCIETIES Dr. Kingsley is a member and regularly attends meetings of many professional societies. In this manner he remains current in state of the art treatment methods for his specialty. A partial listing of these societies follows: American College of Surgeons (F.A.C.S.) American Association for Vascular Surgery Southern Association of Vascular Surgery Southeastern Surgical Congress American College of Phlebology American Medical Association Alabama Vascular Society American Medical Tennis Association Publications & Presentations Word of new findings in medicine is transmitted to colleague physicians in a number of ways, such as articles written by specialists for medical journals. In addition, new and useful information is presented to colleagues at medical meetings. Dr. Kingsley has published and presented over 50 scientific treatises, and has spoken at meetings throughout the U.S., in Germany, and in China. Selected examples of his articles and presentations are as follows: Dr. Kingsley authored a textbook/workbook in the 1980’s titled Ambulatory Health Quality Assurance. Dr. Kingsley performed and published results of the basic research leading to the practice of intra-operative autotransfusion of blood. “Citrate Anticoagulation and On-Line Cell Washing in Intraoperative Autotransfusion in the Baboon” published in the Surgical Forum in the 1970’s. “Heparin versus Citrate Anticoagulation in Autotransfusion” published in the Journal of Surgical Research. He published and presented numerous articles pertaining to vascular problems. “Sclerotherapy and Venous Insufficiency” is a monograph published by the Cleveland Clinic and Wyeth-Ayerst Pharmacy. “Vascular Injuries of the Upper Extremity” was published in the Southern Medical Journal in 2000. “New Standards of Care in Modern Vascular Surgery” was presented to the Birminham Surgical Society in 2001. “Modern Strategies for Limb Salvage” was presented to the Southern Society of Clinical Surgeons in 2000. “Indications and Availability of Carotid Patch Angioplasty” was presented to the Alabama Vascular Society in 2001. “Endovascular Laser Ablation of the Saphenous Vein” was presented to the Alabama Vascular Society in 2002. Dr. Kingsley developed practical experience and expertise in vascular ultrasound in the 1980’s, and presented some of the initial information regarding the use of ultrasound to diagnose carotid artery disease. “Modern Decision Making in Carotid Artery Surgery” was presented to the Pacific Northwest Vascular Society in Tacoma, Washington in 1986. “Carotid Decisions: Color Flow Ultrasound” was presented to the same society in Coeur e’Alene, Idaho in 1988. “Carotid Ultrasound Patterns Predictive of High Stroke Risk: was presented to the Alabama Chapter, American College of Surgeons in 1994. Dr. Kingsley wrote and provided many presentations regarding the use of ambulatory surgery in the United States. He became a spokesperson for the American College of Surgeons, and was invited to speak in Germany and in China. He was President of the American Society of Outpatient Surgeons from 1992 until 1995. “Outpatient Surgery in the United States” presented to Medical Staffs of Chinese Hospitals in Beijing, Chengdu, and Kunming, China in 1993. “Perspectives in Ambulatory Surgery” was presented to a combined meeting of the American College of Surgeons and the German Surgical Society in Berlin, Germany in 1996. Dr. Kingsley introduced many of the ambulatory surgery concepts in practice today. “Office Surgery: 100 + Consecutive Hernia Repairs” was a revolutionary concept presented to the Oregon Chapter of the American College of Surgeons in 1988. “Quality Control of Ambulatory Surgery” was an invited presentation to the Board of Governors of the American College of Surgeons in Chicago in 1988. “Ambulatory Surgery in the United States, An Overview” was an invited presentation to the General Assembly of the American College of Surgeons meeting in San Francisco in 1990. “Ambulatory Surgery, the Gold Standard” was presented to a Symposium on the Operating Room Environment, sponsored by the American College of Surgeons, in Orlando, Florida in 1991. Dr. Kingsley is one of the first physicians in the U.S. to adopt and develop the modern endovenous radio-frequency and laser ablation methods to treat varicose veins, spider veins, and other vein disorder problems. He has one of the largest series of patients treated to date, and regularly reports his innovations and results to his colleagues. He also has developed the Vein Training and Education Institute, for the purpose of training surgeons throughout the United States and other countries. “Endoluminal Laser Ablation of the Saphenous Vein, Technique and Results” presented to the Dornier Laser users meeting, Mirage Hotel, Las Vegas, NV, March 15, 2002. “Venous Ultrasound and Endovenous Saphenous Vein Ablation” presented to the Alabama Vascular Society, Point Clear, Alabama, May 10, 2003. Dr. Kingsley’s latest endeavor is a book which he has been working on for a number of years, titled The Fifteen Minute Patient. This book describes for his patients a method to secure the best medical care in the fifteen minute office time allotment granted by most physicians for patient care.
