        LODGING YOUR FORM

         The following passports applicants must lodge their application in person in London# or Edinburgh##   *



            o   first passport – children aged under 18 do not have to accompany lodging parent
            o   first adult passport
            o   applicants replacing a lost, stolen or damaged travel document
            o   applicants replacing limited validity travel documents (this includes emergency passports) and
                passports which were issued after 20 August 1986 and with an original validity of less than 2
                years
            o   applicants replacing a passport issued prior to 20 August 1986
            o   applicants applying for their first passport on resumption of Australian citizenship

         PLEASE NOTE THAT ALL OTHER APPLICATIONS MUST BE LODGED BY POST TO THE LONDON
         OFFICE.

        CHANGE OF NAME AFTER MARRIAGE, DIVORCE OR DEATH OF SPOUSE

         If you change your surname by marriage or revert to a previous surname because of divorce or death of
         a spouse, you may apply for a gratis replacement of the passport that was current at the time of the
         event. The replacement passport will be issued in your new surname and will have the same expiry date
         as the current passport. To qualify for a gratis replacement, your current passport must have more than
         two year’s validity remaining and you must apply within one year from the date the event took place.
         Your new passport will be issued gratis, however you will still be required to pay the fees for the special
         delivery postage of your new and old passports, please complete the enclosed credit/debit card
         authorisation mandate

        GUARANTOR TO ENDORSE YOUR PHOTOGRAPH & COMPLETE SECTION 12 OF THE FORM

         If you do not know an Australian passport holder who can meet the requirements in Section 12 of the
         PC8, you may use a non-Australian currently employed in the UK in the following approved
         professional/occupational groups who has known the applicant for at least 12 months or since birth:

            o   Chartered/Certified Accountants, Chartered Engineers
            o   Judges, Barristers, Solicitors, Patent Attorneys, Clerks of Court, Bailiffs, Sheriffs
            o   Registered Medical Practitioners, Dentists, Pharmacists, Registered Veterinary Surgeons,
                Registered Nurses
            o   Members of Parliament, elected City, County and District Councillors
            o   Police Officers with at least 5 years continuous service
            o   Members currently serving in the regular Australian or British Defence Force with at least 5 years
                continuous service
            o   Ministers of Religion and Marriage Celebrants
            o   Teachers - full-time, who have been teaching for more than 5 years
            o   Civil Servants/Local Government employees - full-time, who have been employed continuously
                for at least 5 years by their current employer

        PASSPORT PROCESSING TIME
         We aim to issue a passport within ten working days or if you pay a priority processing fee, within two
         working days of meeting our requirements. The two working days commences when the application is
         receipted in our office. If you wish to use this service, please write 'PRIORITY SERVICE' on the front of
         the envelope and place a ‘X’ in the priority fee box on the authorisation slip.

        FEES
         Payment must be made by debit/credit card (except American Express or Diners). As fees are subject to
         exchange rate fluctuations and/or increases in the passport fees, you will be charged the fee including
         postage1 applicable on the date your application is received.              Fees can be viewed at
         www.uk.embassy.gov.au

          form and supporting documents to you by special delivery for which a charge of £5.00 will be made.
