					            NOTES FOR FILLING IN BIRTH CERTIFICATE TEMPLATE
If you cannot relate what is on the birth certificate template to what is on your original birth
certificate it is very likely that you have the abbreviated form of your birth certificate. You
must have a FULL-LENGTH birth certificate. You can obtain this from the office where
your birth was originally registered for a small fee.

Only translate details which are given on your original birth certificate. (i.e. do not add
anything to the French one which is not on the English certificate). Translate words which
can obviously be translated into French e.g. profession of father or mother. Do not translate
proper names e.g. for Hillingdon Memorial Hospital, put L’Hôpital Hillingdon Memorial.

    'NAISSANCE' section

•     No. d'immatriculation à la Securité Sociale = Social Security number - may not be on
      your original birth certificate. Leave this blank if there is nothing on your original
      certificate.

•     No. d'inscription = Entry number/registration number (e.g. you were the 12,305 baby
      registered here). Leave this blank if there is nothing on your original certificate.
•     District d'enregistrement = Registration district. This will be either a name or a number.
•     Circonscription régionale = Area office or Sub-district
•     Région administrative = Administrative region or area

‘DECLARANT(E)’ section (= ‘Informant’ section on your original certificate)

•     11. Nom et prénom(s) (si ce n'est pas la mère ou le père) = name of the person
      registering the birth if it isn't the mother or father (e.g. perhaps an aunt registered the
      baby instead). Leave this blank if there is nothing on your original certificate.

•     12. Qualité = Qualification i.e. relationship of that person to the baby e.g. mother, father,
      uncle, friend, etc.

14. & 16. Signatures
Don’t reproduce these. Just leave blank.

17. Prénom donné après l'inscription, et nom: Usually this is to be left blank. Only fill it in if
your name has officially changed since the registration of your birth.

* Assistants who are adopted can use the same translation but should write ADOPTÉ(E) in
large letters in red at the top. They should fill in the details as far as possible from the
Adoption Certificate.

* Assistants who have changed their name by deed poll.
Please fill in the template as described above. You will have a document explaining your
change of name, perhaps 3 pages long. Please obtain a translation of the full document, not
just a summary. The translation should be done by one of the translators approved by the
French Consulate General. Please contact them for this list. NB. There will be a fee for the
translation.

* If you fall into one of the following categories:

•     Born in another anglophone country, birth certificate in   English and you are resident in the
      UK.
•     Born in another anglophone country, birth certificate in   English and you are not resident in
      the UK.
•     Born in a non-anglophone country, birth certificate in a   foreign language apart from
      French and you are resident in the UK.
•     Born in a non-anglophone country, birth certificate in a   foreign language apart from
      French and you are not resident in the UK.
