2. What hours are you open? Same Day Service Available to Walk-In Customers (for Ohio birth and death records only) Monday – Friday: 9:00 AM – 3:00 PM Regular Office hours: Monday – Friday: 7:45 AM – 4:30 PM 3. What is the cost of a vital record? The cost per certificate is $16.50. Make checks or money orders payable to: Treasurer State of Ohio. DO NOT SEND CASH. 4. How do I place an order for a certificate? There are four (4) ways in which to obtain a vital record in Ohio. Local Office, WalkIn, Mail in, or Credit card/debit card. All four services may be available from the city or county health department office where the event occurred, provided there is no court or legal action. Mail service is available, cost is $16.50, check or money order. For all birth records requiring no amendments (adoption, paternity establishment, etc) and for all death records, please allow delivery time of 3 to 6 weeks from the date the request is received in this office. Any current legal action or correction will require between 3 to 6 months from the date the request is received in this office.
Vitalchek - Obtain a certified copy of a birth or death certificate via credit card using Vitalchek. Additional service charges will be applied to each order. Federal express delivery can also be selected. Turn around is estimated at 3 to 6 weeks from the day the request is received, not including additional time for mailing. 5. How do I obtain a marriage license or divorce decree? The Ohio Vital Statistics Office does not have marriage licenses or divorce decrees on file, only abstracts. For a marriage license contact your County Probate Court where the original marriage license is on file. For a divorce decrees contact your County Clerk of Courts where the original divorce decree is on file. 6. What vital records do you have?
