									The stroke coordinator can provide advice, information and
support for patients and their families throughout their care
pathway to include diagnostic investigations, diagnosis and
treatment. The stroke coordinator works as part of a multi-          Referrals
disciplinary team and will strive to act as patient advocate.
                                                                     The stroke Coordinator should be informed of all individuals

   • Ensure that patients with a stroke and TIA are adequately
       supported throughout their care pathway.
   • To facilitate the stroke teams working together.
   • Ensure that the transfer from hospital to home is
       accompanied by ongoing therapy and information from
       the stroke teams.
   • Facilitate and provide information to both staff and
       patient groups on facilities available for stroke and TIA
   patients.
   • Provide support and advice to patients whilst in hospital
       and after discharge into the community.
   • Facilitate and provide information to both staff and
