A person's religion is "sensitive information" under data protection legislation, so we have to take
care that we issue baptism certificates only to people who are entitled to apply for them.

Please note that a parent can apply for their child's baptism certificate only until the child turns 18;
then the person has to apply for their own.

So we can deal with your application, please fill in this form and send it with your donation to the
address above. Please include a stamped addressed envelope. Thank you!

These questions are to help us find the entry in the baptismal register, and to establish that you
have the right to apply for the certificate (by asking things that you would know, but a stranger
usually wouldn't).
