              Workers’ Compensation Injury Reporting Procedures

If you are injured on the job, follow the steps below:

Step One:

Report your injury immediately to your supervisor. If your supervisor is not available, report
the injury to Public Safety at extension 6206.

Before a claim can be filed with Amerisys, public safety must provide an incident report. In
addition to reporting the injury to your supervisor, you or your supervisor must also report the
injury to public safety.

If this is a life-threatening emergency, notify Public Safety at extension 6666.
Emergency medical care should be sought at the emergency room at
Kennestone Hospital.

Step Two:

The Public Safety team will assess and document the situation and report your injury to HR. If
urgent care is needed, Public Safety will direct you to the proper medical facilities.

                Keep in mind, for injuries resulting from work duties, you can not
                use your personal insurance or your personal physician.

Step Three:

If your injury requires medical attention, HR will file your injury report with the State of
Georgia’s Department of Administrative Services. You will be contacted by the Department of
Administrative Services or Amerisys, the workers’ compensation insurance carrier. Your
contact at Amerisys will direct you to the physician or physicians you need to see to be
covered under the workers’ compensation insurance.

Failure to follow these instructions may result in your medical treatment not being covered
under workers’ compensation insurance.
