					            Substitute Teaching Checklist
Thank you for your interest in being a Substitute Teacher for the La Porte Community
School Corporation.
In order to process your paperwork as quickly as possible, please use the checklist
below to make sure all paperwork is completed. Please see the note** below.

             Application for Indiana Substitute Teaching Certificate. Fill out sections B
             and C. Sign and date at the bottom. We will run your Limited Criminal
             History report at the ESC office.
             Criminal History Information/Employment Eligibility Verification
             LPCSC Application for Employment – Substitute Teacher.
             State of Indiana Employee’s Withholding Exemption and County Status
             Certificate (WH-4).
             Employee’s Withholding Allowance Certificate (W-4).
             $15.00 money order or cashier’s check made payable to State of Indiana
             (cash or a personal check will NOT be accepted).
             $15.00 cash or money order made payable to the LaPorte Community
             School Corporation.
             Copy of your diploma, transcripts, or GED.
             Copy of your driver’s license or similar photo identification.
If you have a valid Indiana Teaching License, please use the following checklist to
make sure all paperwork is completed. The upper portion need not be completed.

             Copy of your Indiana Teaching License.
             Criminal History Information/Employment Eligibility Verification
             $15.00 cash or money order made payable to the LaPorte Community
             School Corporation.
             LPCSC Application for Employment – Substitute Teacher.
             State of Indiana Employee’s Withholding Exemption and County Status
             Certificate (WH-4).
             Employee’s Withholding Allowance Certificate (W-4).
             Copy of your diploma, transcripts, or GED.
             Your TRF# or bring copies of your Social Security Card and Birth
             Certificate in order to fill out the TRF form.
             Copy of your driver’s license or similar photo identification.
Incomplete paperwork will delay the processing of your application and the addition of
your name to our Substitute Teacher Calling List.
**Substitutes will be called in the preferred following order: 1. certified teachers; 2.
applicants with 45+ college credit; 3. applicants with a high school diploma or less than
45 college credits.
