2009 EMPLOYEE RECOGNITION
                        NOMINATION FORM

Hays County would like to recognize its exemplary employees who go “above and
beyond” in their daily work or on a special project.

This form accompanies Section 5.07 of our Hays County Employee Handbook.

Q:    Who can nominate an employee for recognition?

A:    Anyone can nominate an employee for recognition. It can be a co-worker
      nominating another co-worker, a supervisor recommending a staff member, staff
      recognizing their supervisor, or even a private citizen who has experienced
      exceptional service.

Q:    How do I nominate an employee?

A:    Nomination forms have been sent to all department heads and employees. Ask
      your department head for one. Also, nomination forms will be available on the
      Hays County website (www.co.hays.tx.us) and through the Human Resource
      (HR) office in the courthouse, second floor.

Q:    What information should be included on the nomination form?

A:    The form must include a descriptive summary of the specific action(s) or
      behavior(s) that you believe qualify your nominee for recognition. Specify how
      these actions or behaviors contributed to the success of Hays County. Please
      provide statistical information if you wish to show how your nominee made a
      measurable impact on the operations of the organization.

Q:    Where do I return the form?

A:    Completed nomination forms are due in the HR office by Friday, March 6, at

The nomination forms are due in the Hays County HR Office by 5 p.m. Friday, March 6, via
Interoffice mail, fax at 512-393-2227, email at susan.davis@co.hays.tx.us, or hand delivery to
the HR office on the second floor of the County Courthouse in San Marcos. Nomination forms
postmarked by March 6 should be mailed to the Hays County HR Department, 111 E. San
Antonio St., Suite 201, San Marcos, TX 78666.
