Daily service consists of a 30-minute visit including walking, playing, mid-day meal and/or medication,
snacks, potty breaks, and lots of attention.

This signed document is an agreement between The Pink Chihuahua and
______________________________ (Client) for pet care services beginning on _______________ until
revoked in writing. The Pink Chihuahua agrees to provide pet care services to Client in a reliable,
trustworthy, and caring manner.


1. I authorize The Pink Chihuahua to perform pet care services as outlined in Client Information, Pet
Profiles, and Policies and Procedures which shall become part of this contract.

2. I authorize The Pink Chihuahua to obtain any emergency veterinary care that may be necessary during
the time spent with my pet. I accept responsibility for any charges related to this emergency care. I also
authorize The Pink Chihuahua to utilize an alternative veterinarian in the event my primary veterinarian is
unavailable. Every effort will be made to contact the owner prior to obtaining emergency care.

3. The Pink Chihuahua accepts no responsibility for security of the premises or loss if other individuals
have access to the home during the term of this agreement. Pet care will be performed only by The Pink
Chihuahua during all assignments unless prearranged with client (i.e., in the event of The Pink Chihuahua
owner or employee being on vacation, etc.)

4. I agree to reimburse The Pink Chihuahua for any additional fees for providing emergency care, as well
as any expenses incurred for unexpected visits, transportation, housing, food, or supplies.

5. The Pink Chihuahua agrees to provide the services stated in this agreement in a reliable, caring and
trustworthy manner. In consideration of these services and as an express condition thereof, the client
expressly waives and relinquishes any and all claims against The Pink Chihuahua, its employees or assigns,
except those arising from proven negligence of the pet sitter.

6. The Pink Chihuahua will not be liable for the injury, disappearance, death, or fines of any pet with
unsupervised access to the outdoors.

7. Customer will be responsible for all medical expenses and damages resulting from an injury to the pet
sitter or other persons by the pet. Customer agrees to indemnify and hold harmless The Pink Chihuahua in
the event of a claim by any person injured by the pet.

8. The Pink Chihuahua reserves the right to terminate this contract at any time, at its sole discretion;
likewise, client may terminate this contract at any time. There is no term of contract for daily service.

9. It is expressly understood that The Pink Chihuahua shall not be held responsible for any damage to
client’s property, or that of others, caused by client’s pets during the period in which they are in its care.
Client has advised The Pink Chihuahua of all situations, which will relieve it of liability for damage.

10. Fees are earned upon acceptance of pet sitting reservations and are due weekly. No refunds will be
given for cancelled visits once time has been reserved. Clients must give a minimum of 24 hours notice to
receive credit against future visits, which is given at the discretion of The Pink Chihuahua. A fee of $25
will apply to all returned checks. Client is responsible for all costs of collection.

11. I attest to the fact that all licenses and vaccinations required by the State__________, the City in which

1.  Visit times: The Pink Chihuahua will visit at the requested times as closely as possible. However,
    if we are caring for multiple pets, the times may be shifted a little to accommodate our clients. We
    will do our very best to arrive at the appointed times.
2. Inclement Weather: In the event of inclement weather, The Pink Chihuahua has requested on
    your contract the name and phone number of a person living nearby. If The Pink Chihuahua is
    physically unable to reach your home due to impassable roads, please provide us with this
    information so that we can contact this person to request assistance. Your pets’ health and well
    being is our utmost concern and we will contact you as soon as possible to keep you informed of
    these events.
3. Additional Pet Care Assistance And Other Scheduled Services: We all want our pets to have
    all the love and attention they deserve, but please be advised that if there are other persons
    entering and leaving your home, The Pink Chihuahua can not be held liable for any damages or
    problems that may arise as a result. Please inform us at the time of the consultation of anyone
    who may have access to your home while you are away. This includes cleaning services,
    repairpersons, friends, family and neighbors. The Pink Chihuahua does not accept liability for
    other persons who will be in your home during pet care and health services.
4. Vaccinations/Immunizations: The Pink Chihuahua requires that all pets have the necessary
    vaccinations and immunizations before service begins. We may ask to see expiration dates for
    rabies vaccinations.
5. Unforeseen purchases: The Pink Chihuahua will purchase pet food, litter, cleaning supplies or
    other necessary items that contribute to the health and well being of your pet while you are absent.
    We will retain a receipt and the pet owner is responsible for reimbursement of these items. In
    addition a $10 trip fee will be charged to the pet owner.
6. Pet waste: The Pink Chihuahua will properly dispose of all pet waste. We do request that you
    provide plastic bags for this purpose and indicate where you would like these waste bags disposed.
7. Leashes: All dogs will be required to be on leash during outdoor walks.
8. Animal Behavior: Animals behavior can be unpredictable. The Pink Chihuahua does not accept
    responsibility or liability for animal behavior, normal or otherwise, which results in injury to the
    client’s animals. Further, if an The Pink Chihuahua provider is harmed or injured by the client’s
    animals, the client/owner accepts full responsibility for the cost of any necessary medical attention
    required by either the The Pink Chihuahua Pet Care provider or by the animals
9. Fences: Fenced in yards are wonderful playgrounds for our dogs and allow them additional space
    to exercise and play. However, no fence system is totally secure. The Pink Chihuahua does not
    accept responsibility or liability for any client’s animals that escape or become lost or injured, fatal
    or otherwise, when instructed to leave the clients animals in a fenced in area. This includes
    electronic, wood, metal or any other type of fence.
10. Other dogs: We will not permit your dogs to interact with strange dogs. If stray dogs that are off
    leash approach, we will do our best to keep interaction at a minimum and move away from them.
11. House Cleanliness: The Pink Chihuahua will clean up after your pets to the best of our ability.
    Please inform us of the designated area for the appropriate cleaning supplies. If there are accidents
    above and beyond the normal amount anticipated, The Pink Chihuahua will charge a reasonable
    fee for clean up time.
12. Privacy Policy: All of your information will be kept private and confidential. The Pink Chihuahua
    highly respects our clients’ entrusting us with the care of their home and their loving pets. We do

       recommend that you inform a trusted neighbor that while you are away, The Pink Chihuahua will
       be caring for your pets and your home.
   13. Household Emergencies: Please leave the name and number of a trusted maintenance company
       or a person you can rely on to attend to any household emergencies that may arise during your
       absence. This includes but is not limited to; leaking pipes, malfunctioning water heaters and
       heating and air units.
   14. Thermostats: Please leave your thermostat settings within a normal comfortable range (68-78F).
       If the house temperature is outside of this range, The Pink Chihuahua will adjust the thermostat.
       This is to ensure the health and comfort of your pets and during our time of service.

   15. Early Returns/Last minute Changes: It is not unusual for trip plans to change at the last minute.
       However, please understand that The Pink Chihuahua carefully schedules our time to service you
       and our other clients. Therefore, there are no refunds or credits for early returns or last minute
       changes to pet care. Once pet care begins, payment is due for the original dates scheduled.
   16. Cancellations: The Pink Chihuahua requires a full four-day notice prior to the date of the first
       visit. Failure to provide notice in less than four days will result in a $20 cancellation fee payable
       by the pet owner.
   17. Holiday Cancellations: With the exception of severe weather, life threatening emergencies or a
       death in the family, Any cancellations prior to a major holiday; i.e.: Christmas, New Years, Easter,
       Memorial Day, July 4th, Labor Day and Thanksgiving with less than a four day notice will result
       in 50% of the total invoice for scheduled pet care to be paid. We request your understanding that
       Holiday travel is a peak service time for pet care.
   18. Cancellations for Mid-Day Service: A 24-hour notice is required prior to the next scheduled visit.
       Otherwise payment is due for the time originally agreed upon.
   19. Termination of Mid-Day Service: Please provide The Pink Chihuahua with a full week notice in
       the event mid-day service is no longer needed. If a week notice is not provided, payment is due for
       the service originally agreed upon.
   20. Payment: The Pink Chihuahua accepts cash, checks or paypal. Payment is due at the time of or
       prior to the first visit. Please make all checks payable to The Pink Chihuahua.
   21. Returned Check Charges: There is a $25 fee for all returned checks.
   22. Late Payments: There is a 20% late charges fee for all late payments. Payments are considered
       late if not received at the time of the first visit. There is a one-day grace period following the
       date of the last visit before 20% late fees are charged.

                                          CALL HOME POLICY

The safety of your pets is our primary concern. We do everything in our abilities to ensure their safety.
We believe that your pets need to be visited the number of times agreed upon during a day.

While we certainly anticipate that you will be arriving home on the date you provide to us, we understand
that there will occasionally be circumstances beyond your control that will prevent you from returning on
the indicated date. If this happens, please call us and we will absolutely extend past the predetermined
visits to care for your pets.

We ask that you call us when you arrive home. We will also leave a small note card reminding you to call.
This covers an unforeseen event in which you do not arrive home and cannot notify us of this situation.

   Home Care: $10-Our sitters will keep your home and yard in order until you return. We'll
    water your plants, bring in your mail, bring newspapers, turndown blinds.

Hotel Sitting: We can spend time in the room with your dog while you are out at the beach,
having dinner or shopping. Minimum of 2 hrs, $30/hr

Please call us before you arrive, as we cannot book last minute requests for this service. Call to

Holiday Schedule: Book early for the period between Thanksgivings to New Year's Holiday
Season. We generally book major holidays no earlier than a month in advance. For example: we
don’t start booking Christmas until after Thanksgiving.

When a holiday falls on a Friday or Monday we include the whole weekend as a holiday.

Should you require a regularly scheduled midday walk on any of the above holidays, please
schedule in advance. You will be charged a $15.00 visit holiday fee.
