INSTRUCTIONS - Use this guide during a station visitation. Make entries as required and omit entries for items not appropriate to the station. File this with the station visitation report. RATINGS ARE : + , Satisfactory; - , Unsatisfactory or Needs Attention Whenever an item is marked "-" , explain the corrective action taken or recommended and/or reason in the "Remark" space.
