Overview
This document contains the set of templates to be completed when submitting an IP Address assignment request. This set of
templates must be used by NHS Organisations when submitting a request to the NHS IP Registry. The IP address allocations are
administered by BT N3SP, under the supervision of the Authority. Instructions for completing the templates can be found in
Appendix A. Further reading of Appendix B is also recommended prior to completing the templates. N.B Please return all
completed forms to your local N3SP Cluster Service Manager and mail to n3deliverytechteam@bt.com
Please read Appendix B for information on Voice over IP (VoIP) allocations
If this is a request for additional Class “A” addresses, please disclose this in section 5.

Appendix A: Supporting Guidance for Completing this Form
1. Organisational Details
To contain details of the NHS Organisation where the requested IP address space will be deployed
BT Service Identification Number (SIN) - Original NHSnet BT Healthnet Site identifier
N3 Access Agreement Number - Unique Identifier for registered N3 sites
OCS/NAC Code - Organisational Code Service details or National Administration Code

3. Request Summary
Total Headcount Size - Total number of users within the organisation
Total Address Space Required - Total number of host addresses required (Immediate and Year 1&2 requirement)
Immediate Requirement - Total number of host addresses that will be deployed immediately.
Immediate Subnets - Total number of subnets required immediately.
Year 1 Requirement - Total number of host addresses that will be deployed in subsequent Year 1.
Year 1 Subnets - Total number of subnets required for Year 1.
Will NAT be Used (YES/NO) - Indication as to whether NAT will be used at the NHSnet interface.

Appendix B
Conditions of Use of NHS Registry Allocated Address Space
Address space allocated by the NHS Registry must be used in accordance with the NHS IA’s guidelines for address deployment.
The Authority reserves the right to revoke address space allocations, where address space has been deployed inappropriately.
Allocations are based upon the capacity available within the private class A address space. Therefore, there is a necessity to
ensure equity is maintained across the NHS.
Guidelines for Address Deployment
1. NHS Registry address space must not be deployed behind a NAT device. Organisations wishing to migrate away from NAT
will be permitted a reasonable period of time to conduct their address migration in a controlled manner before NAT must be
removed. Organisations that wish to maintain NAT must only deploy the Private Class C address range (192.168.0.0 –
192.168.255.255) within the LAN.
2. Organisations must ensure address space is utilised in the most efficient manner possible. The NHS IA regards the use of
large subnets as a mechanism to assist in the management of the LAN infrastructure as a wasteful approach.
3. Organisations must ensure subnet masks are utilised to achieve a sensible and efficient use of address space in relation to
the physical number of devices and users within the network.
4. The Authority regards the use of NHS Registry address space for the accommodation of 3 rd party management links across
private circuitry as inappropriate.
5. In most VOIP/IPT deployments, the voice and data traffic remains separated through the use of Virtual LAN (VLAN) technologies;
therefore the address space for the two services is not required to be contiguous to each other. In the absence of an N3 VOIP/IPT
solution, organisations that wish to deploy VOIP and IPT locally should adopt addresses from the 192.168.x.x range. N3SP will
allocate addresses for the new service once a national N3 VOIP/IPT solution becomes available.
