The content of your answers, as well as the manner in which you express yourself will be taken into consideration
when reviewing your English Waiver request. In addition to quality of content, attention will also be focused on
correct spelling and grammar. Be sure to complete the form in its entirety and ensure you have signed and dated
                                                   the form.

Please print neatly in the space provided for each response. Illegible submissions
will not be reviewed. If you use a computer word-processor, please number each
response and attach your printed pages to this completed form.
1. In your own words, please indicate why you feel the request for English proficiency should be
waived. Indicate your experience with the English language in a work or educational setting.

2. Please provide information regarding all English language preparation/courses you have taken. If the
courses were completed at an English language school outside of Canada, you are required to provide
written proof directly from the institution that the language of instruction was in English. If you have
attended official ESL courses, please submit official transcripts outlining your level of achievement in
each course/program.

3. Indicate scores you have received on ANY English Proficiency test/exams which you have taken.
Official scores/reports must be submitted to the Registrar’s Office.

4. Please attach a brief, one-page letter indicating why you wish to attend King’s University College.

You will be notified in writing of the decision regarding your Waiver Request. Only in exceptional cases may an applicant request a review of
their decision. Such a request must be made in writing to the King’s University College Admissions Committee within two weeks of the issuance
of the decision. In order to have the Admissions Committee review the original decision you must provide evidence of a substantive nature that
one of the following affected the decision: Senate-approved procedure not followed; Senate-approved policies governing admission or
readmission not followed; unfairness or bias in Admissions Committee.
Your signature indicates that all responses on this form and any accompanying pages are factual and
were prepared entirely by you, and that all documents submitted in support of your waiver are original
